RE:           Information for PERAC’s 2000 Annual Report

DATE:         March 29, 2001


I have attached a copy of some of the information about your board that will be included
in PERAC’s 2000 Annual Report.

Your board address, telephone number, fax, and board meeting information should be as
of the current date.

However, the names of board members and the board administrator should be as of
December 31, 2000.

Please validate the information. If you have corrections to make, please fax them to
Sarah Kelly at 617-628-4002 or e-mail them to her at Sarah.Kelly@state.ma.us no later
than April 10, 2001.

If you do not submit corrections, the information as you have received it, will be included
in the Annual Report.
