This pamphlet is a consolidated index of all current U.S. Army Recruiting Command (USAREC) publications and blank
forms issued.

3. Explanation of abbreviations
Abbreviations used in this pamphlet are explained in the glossary.
4. Publications general
a. (J) - USAREC/FORSCOM (U.S. Army Forces Command) or USAREC/FORSCOM/TRADOC (U.S. Army Forces
Command and U.S. Army Training and Doctrine Command) joint publication.
b. The letter designations A, B, C, D, E, and Y shown in the index (apps B through E) indicate the level to which the
publication is intended for or was distributed. In addition, “Special” is shown for certain publications and indicates
other than a standard distribution. The formulas for special distribution are as prescribed by the originating staff office.
c. The following is the distribution scheme:

e. Complete files of all publications are neither required, nor authorized, for all elements of this command; however, if
the activity is indicated as a recipient, the publication will be maintained by the activity concerned. Having access to the
electronic copy is sufficient.
f. HQ USAREC staff elements preparing publications will indicate distribution desired by use of the appropriate
symbol listed in table 1. Proponents desiring an “A” distribution must coordinate publication with the G-3, Enlistment
Eligibility and Processing Division.
g. The U.S. Army Special Missions Brigade (SMB), U.S. Army Medical Recruiting Brigade (MRB), recruiting brigades
(Rctg Bdes), medical recruiting battalions, Rctg Bns, and MRCs are not authorized to publish their own publications. If
a publication is required, it should be submitted to HQ USAREC, Assistant Chief of Staff, G-6, ATTN: RCIM, 1307 3rd
Avenue, Fort Knox, KY 40121-2725. At that time, it will be staffed with the appropriate proponent for review.
5. Forms general
a. All USAREC activities, except Rctg Cos, MRCs, and RSs, will maintain a 120-day stock level of applicable USAREC
blank forms. RSs, MRCs, and Rctg Cos will maintain a 60-day stock level.
b. The SMB, Rctg Bdes, MRB, Rctg Bns, and medical recruiting battalions will maintain either DA Form 479 (Publica-
tion and Blank Form Stock Record Card (Vertical File)) or DA Form 479-1 (Publication and Blank Form Stock Record Card
(Visible File)) as stock record cards for USAREC, Department of the Army (DA), Department of Defense (DD), Standard
Form (SF), and Optional Form (OF) blank forms (where stocks are maintained). An automated program containing the
same information as the DA Form 479 or DA Form 479-1 may be used in lieu of the manual system.
c. Stock record cards will be kept current and reviewed at least semiannually by the records management coordinator.
A statement will be prepared at the time of review and kept on file.
d. To compute your monthly usage, follow the steps below:
(1) Keep an accurate count of your usage for approximately 3 months.
(2) Divide your usage by 3. Used 800 forms in 3 months; 800 : 3 = 266.66; round up to 270. Your monthly usage in
this case would be 270.
e. There is no need to maintain stock or stock record cards on items that have been electronically generated and the
paper copies are no longer published or being used.
f. The SMB, Rctg Bdes, MRB, Rctg Bns, and medical recruiting battalions are not authorized to publish their own
blank forms. If a form is required, it should be submitted to HQ USAREC, ATTN: RCIM. At that time, it will be staffed
with the appropriate proponent for review.
6. Initial distribution
a. The SMB, Rctg Bde, MRB, Rctg Bn, and medical recruiting battalion commanders will establish, if not already in
effect, an account with the St. Louis Publications Center. The Rctg Bn and MRC account will be for required publications
and blank forms at the RS and Rctg Co level.
b. The SMB, Rctg Bdes, MRB, Rctg Bns, medical recruiting battalions, and MRCs receiving DA, DD, SF, and OF
blank forms from St. Louis Publications Center will maintain a 120-day stock level of these forms.
c. The SMB, Rctg Bde, MRB, Rctg Bn, medical recruiting battalion, and MRC commanders are required to review at
least annually, their DA Form 12-series and submit a verified copy to HQ USAREC, ATTN: RCIM.
d. Table 1 shows the breakdown of initial distribution for USAREC publications. The SMB, Rctg Bdes, MRB, Rctg
Bns, medical recruiting battalions, and MRCs will submit a memorandum of justification on a case-by-case basis for
changes to the initial distribution scheme.
7. Resupply
a. The SMB, Rctg Bdes, MRB, Rctg Bns, medical recruiting battalions, MRCs, and HQ USAREC staff elements will
submit requisitions for resupply of USAREC publications and blank forms to HQ USAREC, ATTN: RCIM, by use of DA
Form 17 (Requisition for Publications and Blank Forms). Requisitions will be consolidated as much as possible. Requi-
sitions requesting more than five copies of any publication must be justified in block 2 on DA Form 17; if not, the
requisition will not be honored. Requisitions for blank forms must have block 7c (Unit), DA Form 17 filled out. At table
2 is a list of all units of issue. A shipping date of 7 days from date of receipt of the requisition will be established, unless
circumstances prevail which require emergency action. In such cases, the special requisition must contain a statement
of justification. Requisitions will be processed and shipped to account holders and staff elements only.
b. To acquire necessary U.S. Military Entrance Processing Command publications or blank forms, requests will be
forwarded through Rctg Bns to their respective servicing Military Entrance Processing Station.

8. Excess forms or publications
On the occasion that there are excess forms or publications within your organization, the following procedures will be
followed:
a. Inventory and verify that stocks are actually in excess to your needs. All excess forms or publications will be
identified by amounts on hand.
b. Contact HQ USAREC, ATTN: RCIM, DSN 536-0034 or commercial (502) 626-0034.
c. You will be given disposition instructions pertaining to each form or publication identified. Do not forward excess
forms or publications to this headquarters before receiving disposition instructions.
