There are 3 types of Institutional Excuse Forms: 1. Academic Institutional Excuse Forms - We sign these in the CENS Dean's office...please bring them to me or put them in my box in BIO 132. These are used for required course or academic program-related activities, not club-related activities. Fore example, class field trips that are part of the course requirements; presentation of a research paper at a conference or symposium that is approved by the research/academic advisor. 2. Extra-Curricular Institutional Excuse Forms - We cannot sign these, rather approval must come from the Dean of Students (Not from the CENS Dean's Office). We do not have these forms in the CENS Dean's Office, but rather they must be picked up at the Office of Student Life, University Union, Room 105. These are used for club-related activities. For examples, annual regional or national club meetings the the NAU club is attending or sponsoring. 3. Athletic Institutional Excuse Forms - We cannot sign these, either, rather approval must come from the Department of Athletics and we do not have these forms in the CENS Dean's Office. The are used to excuse student for NAU athletic events in which the students participate as NAU athletes. If you think you have a need for an Institutional Excuse, but aren't sure which one you need, please ask before going down the wrong path and having to re-do it.
