                                                 EXECUTIVE SUMMARY

MicroTek, Inc. is a highly competitive, not-for-profit electronic manufacturing company that specializes in the production of
custom cable assemblies and wire harnesses. Headquartered in Chicopee, MA, MicroTek operates out of a well-
designed, state-of-the-art building that is fully accessible and presents a highly valued image of the company and its
employees. MicroTek employs a very diverse workforce of one hundred and fifteen employees. Of that number, fifteen
individuals receive employment services contracted through the Department of Developmental Services,
Holyoke/Chicopee Area Office.

Findings of the current licensing and certification survey were based on a review of employment services provided to five
individuals. These employees held the entry level position of Assistant Assembler with the long-term goal of acquiring all
of the needed skills to qualify for an Assembler position with the company. This survey was a full licensing and
certification review that evaluated employment services across all Quality of Life Areas and Outcomes for the
Organization.

Since the last review, completed in May 2008, MicroTek was engaged in implementing a number of calculated business
strategies to address the concerns of a downturn in the economy in the same year. The agency employed a number of
interim measures to strengthen the company’s capacity to remain competitive, satisfy customer needs, and maintain
quality services to individuals. Several of the measures taken by the agency included cutting company overhead costs
and implementing procedures for voluntary layoffs that did not include Assistant Assemblers. As part of an ongoing
process, the agency continued to evaluate where production efficiencies were possible without compromising product
quality. To that end, additional areas of product development were identified that could be broken down into smaller
steps. These efficiency measures produced additional jobs for Assistant Assemblers and the workforce in general.

A significant strength of MicroTek that has been demonstrated over time is its commitment to being a highly competitive
business entity informed and directed by a strong value-based Mission Statement, Core Values and Guiding Principles.
The agency has been very successful in developing a corporate culture where individual differences and diversity are
recognized, valued and supported. Assistant Assemblers worked in a fully integrated setting, earned at least minimum
wage or higher, and received benefits in accordance with their position and employment status with the company. This
included paid holidays, personal time, medical leave, annual performance evaluations, and opportunities for
advancement.

Another consistent finding of the review was the agency’s commitment to supporting a strong human rights system.
MicroTek had a very committed human rights committee that was comprised of many longstanding and well-informed
members. The committee maintained compliance with composition requirements and met at least five times per year. A
review of the committee minutes illustrated the degree to which individuals’ rights and personal well-being were
discussed and were central to the committee’s concern, which often times and to the benefit of individuals extended
beyond the realm of employment supports provided by MicroTek. The Human rights committee and trained human rights
officer continued to be effectively supported to carry out their roles and responsibilities by the agency’s longstanding and
very skilled human rights coordinator. Individuals received annual human rights training that covered matters regarding
workplace rights and responsibilities. Routinely, guardians and involved family members also received human rights
information and contact numbers. Overall, the agency’s human rights training and oversight systems were fully effective
in affirming and protecting the rights of individuals who received DDS-funded services.

MicroTek continued to demonstrate its commitment to supporting individuals to grow and experience success personally
and as employees of the company. Support strategies were thoughtful and individualized, and people were able to benefit
from their work experiences. In total, people were effectively supported to work toward accomplishing various skill levels
that would bring them closer to achieving positions as Assemblers. Individuals were similarly supported to work on other
areas of personal growth that included overcoming personal challenges. Support teams were creative, knowledgeable,
and fully supported each person’s achievement and success in what they were working on.

Microtek placed a high priority on promoting workplace safety. The agency effectively used several mechanisms to
ensure that all local codes, ordinances and OSHA requirements as well as external inspections were current and in place.
All machinery was properly handled by employees, while using specific procedures when machines were not in use, or
receiving routine maintenance and repairs. All facility safety features such as exit signs, auxiliary lights, fire extinguishers
and the contents of the company’s first aid kit were present and subject to routine inspections. Safety assessments were
completed for each person that covered a wide variety of work situations to determine what support a person might
require to perform different jobs.

Staff were current in all of the required trainings that included first aid, CPR, fire safety, mandated reporting, and MAP
certifications. Staff were diligent and well attuned to individuals’ health-related concerns, including dietary needs that
required supervision during breaks and lunchtime to minimize potential choking incidents. The agency also on a regular
basis updated information on medications or changes in the person’s health status. These practices and methods of
oversight ensured that individuals’ health and safety needs were continually and proactively addressed.

In conclusion, MicroTek has continued excel in providing high quality employment services to individuals across all
Quality of Life Areas over numerous Survey and Certification reviews. It was clearly evident that MicroTek and its staff
are genuinely invested in people’s success and their contributions to the company. As a result of the current survey
findings, MicroTek has earned its sixth consecutive Two-Year License and is certified with Distinction.

Principles: Support and affirmation of individual rights are safeguards for all people as citizens of the United States. These rights are
framed in the Constitution, Bill of Rights, and Federal and State statutes. They include such freedoms as the rights to privacy,
free speech, and due process. The actualization of individual rights is especially important to people with disabilities because
they have not always been afforded these most basic of guarantees and may need support to exercise their rights. In order to
have their rights affirmed, individuals first must be respected, valued, and treated with positive regard.

Principles: Being safe, secure and healthy provides a foundation that enables people to have a truly fulfilling life and do the
things that are important to them. As with rights, certain universal things contribute to the well-being of everyone
in society, regardless of who they are and their personal circumstance. These universals include having routine
physical examinations, wearing seat belt, or installing smoke detectors. Beyond the universal safeguards,
personal well-being is supported by recognition of people's skills and needs as balanced against their lifestyle
choices. Services are designed to assist people to be secure, achieve and/or maintain good health, to make
decisions that keep them out of harm's way, and to ensure a prompt response when people's safety, security or
                 health are compromised.

                                            OUTCOMES FOR THE ORGANIZATION

Principles: Positive outcomes for individuals are the result of many factors, not the least of which is a strong, coherent organizational
structure. For positive outcomes for individuals to be sustained over time, across all settings and by all staff, agencies need to
create an infrastructure and culture that supports quality and safeguards individuals.

Principles: Being able to direct the course of one's life is something for which all people strive. Self-determination is expressed in all
aspects of people's lives, both in little things such as deciding when to get up as well as in major life decisions that may have a
deep, long lasting effect such as deciding on a certain career or moving to a new community. The type and amount of support
people need to take charge of their lives varies greatly. For some, even making everyday choices is a new experience and they
need much guidance in learning how to make both everyday and major choices. For others, periodic check-ins and discussions
are needed to keep them on course and to make sure they have an understanding of their options and of the consequences of
their decisions. What is important is that people have all the information they need to make decisions, that they are empowered
and supported to take action, and that they understand how their decisions affect their lives.

Principles: Membership in community life supports people's sense of belonging and provides opportunities for citizenship, friendship and
valued roles in society. Everyone has unique, identifiable ways that he or she is connected to the community and to other
people who are important to them. Being a part of community life means that people are using all the resources and amenities
that others use. Being connected also means that people have other people in their lives who are important to them, who are
there for them when needed and with whom they can share their thoughts and concerns. It also includes having a place in
society, a place that other people recognize and value. It may mean helping out neighbors and friends when needed or having
talents and gifts that are recognized and shared with other people.

Principles: While developing one's own goals is a critical first step in making decisions about one's life, achieving personal goals and
developing the skills to be as independent as possible give individuals a sense of purpose, direction and self-fulfillment in their
lives. The strategies and support used in assisting people to reach their goals takes on many forms and should be tailored to
people's unique skills, needs, preferences and style of learning. It includes helping people to learn from both planned as well as
unanticipated events in their lives. While there is much to be proud of in acquiring the skills to do things independently, it does
not mean that people are independent in everything they do. There is the joy that people feel in accomplishing things together
and the feeling of self-worth that comes with completing something as independently as possible.
