							                INSTRUCTIONS FOR FILLING OUT SKI TRIP FORMS


The following forms MUST be returned to our office no later than November16th:

*      Ski Trip Registration Form
*      Medical and Liability Release – FPC Youth Ministry
*      Copper Mountain/Winter Park Release
                     *This is on the form that reads “2007-2008 Adult Lesson Registration” at the top.
                     *You do not need to fill out the top portion unless you are buying a lesson. You
                     only need to sign and date the Waiver portion.

          And, ONLY if your student is taking ski or snowboard lessons,
          • 2007-2008 Adult Lesson Registration
                     *This is entitled “2007-2008 Adult Lesson Registration” and is at the top of the
                     Copper Mountain/Winter Park Release

The following additional forms are only for your information about the trip:

          •   Ski and Snowboard Lessons
          •   Packing List
          •   Optional Travel Insurance
