Customer chooses which option 3 -5-10 etc’ Customer then is sent to the shopping cart. Will have 10 different options and prices. Customer places their information: Input Name Address SSN# ETC. Input Forms will also have up to 10 Lines of input for up to 10 different companies. Depending on the option they choose Example 3-5-10 There will be 10 company option choices total. Input to include, Company, Name Address, Phone # Fax# SUBMIT – PAYMENT - APPROVED This information will then filter through to 2 sources - 1St Customer information to be sent to an auto responder service. (Example Aweber) 2nd source to MYSQl Database Which will be exportable in CSV format so it can be uploaded into company form output printer each form and print out with same client info on each page with each page having a different company’s information on each pages letterhead. SEE ATTACHED EXAMPLE After using a shopping cart service type of function. Information will then submit on the online server for CSV export.
