Attachment 3 OLF guidelines for FILLING IN DECLARATION FORMS FOR HAZARDOUS WASTE February 2004 Page 2 The purpose of these guidelines is to give the producers of waste the necessary codes and information for filling in declarations. The guidelines are an attachment to “OLF’s common guidelines for waste management in the offshore industry”. A completed declaration form shall be sent together with the manifest accompanying the cargo being returned to shore. The entire original declaration form shall be sent ashore. The party possessing the waste shall retain copy 5 (the company decides whether it is to be kept on the platform or at the base), but the sender (e.g. storekeeper) shall make a photocopy for his/her own file offshore. The hazardous waste shall be properly packed. The packaging shall contain the form’s declaration number, and in the case of hazardous cargo, the UN number and hazardous waste note(s). Hazardous waste classified as hazardous cargo shall be stored and transported in UNapproved packaging. Example of how to fill in the declaration form: Field 1 Organisation number. Operator’s organisation number (number in the Register of Business Enterprises). The number of the relevant local entity shall be used. The invoicing address need not be stated. Field 2 Producer of the waste complete with full address (onshore address). Field/base/onshore facility operator. The producer of the waste stated here must exist in the Register of Companies. Field 3 Contact. Position of the responsible person filling in the form (operator’s own employees such as storekeeper or supplier employees onshore/offshore). Field 4 Telephone number. Telephone number of relevant position on the installation, e.g. storekeeper or onshore contact (supply base or operations department). Field 5 EWL code and waste substance number. Note! EWC = EWL in the hazardous waste regulations applicable from 1 January 2003. Use the enclosed table or seek advice from experts/top expertise in your own organisation or at a supplier of waste handling services. Field 6 Number of kilos/litres and number of items. The quantity is usually stated in total kilos (e.g. everything with net weight greater or equal to 1 kg/l, but oil is given in litres (net weight <1)). The weight shall include the packaging with the exception of bulk/tank deliveries (bulk is the transportation of solid substances in a tank). Field 7 Further description. Describe type of waste, and the name of the original location of the waste (platform/rig/ship). If an HSE datasheet exists, enclose it and write “see enclosed datasheet”. In case of a mixture of substances, such as process water or the like, describe the contents of the mixture and percentage distribution and enclose an HSE datasheet of all the chemicals in the mixture. Field 8 Transportation classification. ADR class, packaging group and UN number shall be filled in (ADR digit is changed to packaging group). Use the enclosed table or, if possible, check the HSE datasheet or seek advice from experts/top expertise in your own organisation or at a supplier of waste handling services. Field 9 Date and signature. A qualified person, e.g. storekeeper, who quality assures the form before transportation. Examples of waste that is NOT special waste a. Examples of waste to be separated as industrial waste Waste type Small disposable batteries Comments Normal alcaline- and brownstone batteries. Page 4: The table shows a PROPOSED classification (the most common) as e.g. the flash point will determine whether it is to be packaging group I, II or III. Previous ADR digits have been replaced by packaging group. I is for highly hazardous waste, II for hazardous waste and II for less hazardous waste.
