All documentation for absences must be attached and turned in with this request. Phone calls do not excuse a student’s absence from school. The following documents are required to excuse an absence: Medical appointment Dental appointments Court dates Pre-approved absences of educational value. Example: visiting a college or a military recruiter Death or illness of a family member Other reasons deemed acceptable by the Credit Review Committee. Parent notes do not count as legal documentation
