E-rate Form 486 Fall 2008 Form 486   Receipt of Service Confirmation Form Deadline is 120 days after July 1 (service start date) or 120 days after receipt of your Funding Commitment Decision Letter (FCDL) Be aware of:   If you do your Form 486 before July 1 please make sure that you have a technology plan approval letter so you can “legally” certify that you have an approved technology plan. There is an extra certification if you submit the Form 486 before July 31 (Block 2, 6a) Components     4 Blocks Block 1 - Billed Entity Information Block 2 - Early Filing Information and CIPA Waiver Requests Block 3 - Service Information Block 4 – Certifications and Signature Tips for using the online form    If you find you can’t complete the form in one session, use the Save and Exit button. You will get a Form 486 Form Number and a security code. Print this page or record these numbers. You will need them both to continue an incomplete form Disable any Pop-Up Blocker. You won’t be able to successfully complete and submit the form if pop-ups are blocked. Do the Online Form 486      Go to: http://www.sl.universalservice.org/menu.asp Under Form 486, click on “Create Form 486” Enter your Billed Entity Number (BEN) to start a new Form 486, then click the Find button Verify name and address information Click Start New Form 486 button Block 1 - Billed Entity Information      Applicant’s Form Identifier (You create this yourself to help you identify this form.) Basic info: Entity Name, Address, etc. Remember to select the correct funding year in number 3. Contact person Information Note: It is best to use email address as the preferred method of contact. Block 2 - Early Filing Information and CIPA Waiver Requests  6a – Only check if you are filling out the Form 486 before July 31. 6b – Only if you are new to E-rate and are still working on CIPA requirements. This will apply to very, very few Maine schools and libraries 6c - Only if you are redoing a 2004 application   Block 3 Please Note:  In the online Form 486 you do Block 4 Certifications before you get to Block 3.  Then you’ll do the Block 4 Signature Block 4 – Certifications For Basic Telephone Service (local, long distance, cell)    For the first certification, #8: Check the box next to # 8. Then select “Other” at the bottom of the drop down menu marked “Please select TPA” Then type “None” in the box below Block 4 – Certifications (For ATM or other services)    If your Form 486 is for Internet or a Telecommunications service that is not basic phone (like ATM) For the first certification: Check the box next to # 8. Select your Technology Plan approver from the drop down menu. It should be either Maine Department of Education or the Maine State Library Block 4 – Certifications and Signature Read then check certifications for 9 and 10 For number 11  Check “a” if this Form 486 is for Internet to indicate CIPA compliance  Check “c” if this is for basic telephone service  Certifications “d - g” are for consortia so don’t check those  Click Next  Block 3 - Service Information     Use the information from your Funding Commitment Decision Letter (last page). In the Online Form, once you fill in (B) and (C) (A) and (D) will fill in for you. Use the format 07/01/2008 for your Service Start Date (or use the calendar icon to select) Click Next Block 4: Signature    Make sure the Printed name of authorized person is the name of the person who has the PIN. Fill in the rest of the address, phone and email info Click Next Submit Form 486     Click the Print preview button in the top right corner of the online form. Verify that the information is correct The click the submit button Once submitted you will then have to certify the Form 486 Certify Form 486 You will see three buttons  Certify on Paper  Certify Online Now  Certify Online Later  Select Certify Online Now if you have a PIN  If you don’t have a PIN you must send in the Certification on paper with your handwritten signature (sometimes called a “wet” signature) Certify Form 486       The certification page will open another window You will see the list of certifications made earlier in the form. You will also see at number 7 an “empty” box to enter your PIN. Type your PIN in the box and then check the compliance box You will get a pop up with a “Cert Id” Close this box Certify Form 486     You will see the Cert Id where you typed in your PIN. Print this page and attach to the rest of the Form 486 that you printed before. There is no print preview or print button on this page. Just go to: File  Print Once you have a copy for your files you are done and you can close all windows. Document Retention   You must keep copies of all forms, correspondence, etc. with USAC/SLD for five years after services are received.
