            2008 Parental Permission Form for Practices
                                            Required for all participants under 18 years old

PRACTICE Description
    o PRACTICES @ Lake Parsippany, Parsippany, NJ
    o Weekday 6PM (Tuesday, Thursday) and weekend 10AM (Saturday, Sunday) sessions run about 90 minutes between April and
      October
    o Practices include 5-10 minutes of on-land warmups, which includes strenuous stretching and calisthenics and over an hour of on-
      water paddling
    o Practice sessions are in 10 or 20 paddler dragon boats
    o Participants may also serve as dragon boat drummer or steerer
    o We require all participants under 18 years old to wear a life jacket
    o NJDBC provides US Coast Guard approved type III life jackets and paddles for all participants
    o Participants will be asked to leave the practice if their behavior creates an unsafe environment or disrupts the session. Such
      behavior may cause the participant to lose their membership benefits.
    o Practices include adult participants
    o Membership into NJDBC as a youth under 18 is $25 a year (write checks to: "NJ Dragon Boat Club")
    o All practice sessions are adult supervised.
    o Youth members are added to the NJ Dragon Boat Club email news group and Youth chat group “teamaiya@yahoogroups.com”.

PARENTAL/GUARDIAN RELEASE FORM for all 2008 NJDBC practices

    o As parent or legal guardian of the above named participant, I certify that said person has my permission to attend and participate in
        the above listed event/race with the NJ Dragon Boat Club and/or affiliate club teams.
    o   I also certify that I am responsible for how or with whom the above named participant gets to and returns from the event/race.
    o   I understand it is the responsibility of the above named individual to NOT participate in any event/race they deem as unsafe or
        dangerous.
    o   No liability whatsoever is assumed or will be exercised by the undersigned.
    o   I give permission to the program leaders to render First Aid, should the need arise in the care of the above named participant. In the
        event of an emergency, I also give permission to the medical personnel, selected by the adult leader in charge, to hospitalize,
        secure proper anesthesia, order injection, or secure other medical treatment, as needed and should medical care be necessary, I
        authorize medical personnel to release information as to treatment, status, and condition to the adult leader in charge.
    o   I understand I am responsible for any damage done by the above named participant to NJ Dragon Boat Club, Metro Event
        Enterprises, Lake Parsippany or event/race property or equipment.
    o   The above named participant agrees to abide by the NJ Dragon Boat Club Agreement/Code of Conduct and represent the team in a
        mature manner. The participant also agrees to comply with the rules of the trip set by the Coaching Staff and the chaperones
        working the trip. Any violations could result in immediate parental or guardian notification and possibility of removal from the trip. In
        case of removal, it will be the parents'/guardians' responsibility to provide for supervision of the participant for the remainder of the
        trip or pay the transportation home if supervision is unavailable.
    o   I understand that participating in this event involves recreational activities and competition in which can lead to accidents and
        involves a certain degree of risk. I have carefully considered the risk involved and given the participant my consent to participate in
        the dragon boat event/race. I hereby agree to the participant's participation and waive all claims against the leaders of this trip and
        officers, agents and representatives of the NJ Dragon Boat Club, Metro Event Enterprises, Lake Parsippany Property Owners
        Association. This also means to release, indemnify and hold harmless these individuals and groups from any and all li ability for
        claim or injury or loss sustained by my child's participation in this event/race.
    o   To the extent that the above named participant is a minor, I understand I will assume all risks, and will agree to indemnify the
        leaders of this trip and officers, agents and representatives of the NJ Dragon Boat Club, Metro Event Enterprises, Lake Parsippany
        Property Owners Association for any claim arising out of the trip or arising based in whole or in part in a failure of adequate
        supervision brought by or on behalf of the minor now and in the future.
    o   I understand additional parental permission slips are required for each event/race in order for the participant to take part in them.

                                           Required for all participants under 18 years old

EVENT or RACE Activity Description
   o Events are usually one or two day events.
   o One day events may require an overnight stay at a local hotel
   o All participants are expected to pay their equal share of the housing, food and travel
   o Each event or race has a set fee for participants, used towards paying the event's entry fee
   o Participants not a NJDBC-member pays an additional $5-25 non-member fee for each event
   o For the events requiring overnight housing, NJDBC will help arrange for hotel room booking. Each participant is expected to pay
       their share of all costs. Rooming assignments may require the participants to use their sleeping bags to promote cost savings. We
       will assignment rooms by gender for the youth participants. All youth participants expected to adhere to 11PM curfew.
   o   Transportation to and from these events may be driven by other youth participants
   o   All Events and Races will be supervised by adults
