The Shriver Award for Humanitarian Service is awarded by the National Peace Corps
Association to a Returned Peace Corps Volunteer who continues to make a sustained and
distinguished contribution to humanitarian causes at home or abroad. The award was named to
recognize the tremendous contributions of the first Peace Corps Director, Sargent Shriver, in the
founding and development of the Peace Corps.

To nominate an individual for this award, please complete this form and send it along with a
nomination letter and supporting materials (see below) to:

Your nomination letter should include the following:
   1. Summary of your nominee’s background and career, highlighting the significance to
      humanitarian service of the activity that you feel qualifies your candidate for the Shriver
      Award.
   2. Distinctive accomplishments and personal qualities of character and idealism that set
      your nominee apart and those qualities that suggest that he/she may serve as an
      inspiration and role model for the entire Peace Corps alumni community.
   3. Along with your letter of nomination, please include a resume, bio or curriculum vitae for
      the person being nominated. You many also wish to include letters of support, copies of
      articles written about the candidate, or other documentation you feel may be appropriate.
