2nd Sample Talent Release Form
Talent Release FOR VALUABLE CONSIDERATION, including the agreement to produce the motion picture currently entitled “HEARTS IT IS,” I hereby irrevocably grant to Hearts It Is, Pengin Productions, its licensees, agents, successors and assigns, the right (but not the obligation), in perpetuity throughout the world, in all media, now or hereafter known, to use (in any manner it deems appropriate, and without limitation) in and in connection with the motion picture, by whatever means exhibited, advertised or exploited: my appearance in the motion picture, still photographs of me, recordings of my voice taken or made of me by it, any music sung or played by me, and my actual or fictitious name. On my own behalf, and on behalf of my heirs, next of kind, executors, administrators, successors and assigns, I hereby release Pengin Productions, its agents, licensees, successors and assigns, from any and all claims, liabilities and damages arising out of the rights granted hereunder, or the exercise thereof. I am the parent or legal guardian of _______________________. I hereby irrevocably consent to the foregoing grant and agreement. I understand that state law requires me to attend each of my child’s performances, and I testify that this information was provided to me by the producer of this film. However, due to circumstances beyond my control, I may or may not be able to attend each performance, and in these instances, I agree not to hold the producer responsible. I also give permission for my child to work past 9pm on production nights when necessary. I will not assert or maintain against Pengin Productions any claim of any kind or nature whatsoever, including, without limitation, those based upon child labor laws, in connection with the exercise of the permission herein granted. Tips... -Notice in the parent's form how I got around child labor laws. The 15 year old that I worked with didn't need a work permit (in Indiana) because entertainment employment is an exception. Always check with your state's child labor laws when working with minors.
