       “OFFICIAL TRANSCRIPT OF GED PRACTICE TEST RESULTS” FORM

      NOTE: GED Practice Test scores earned prior to January 1, 2003 are not
      recognized as meeting the criteria for issuing an Official Transcript of GED
      Test Results form.

After January 1, 2003, all persons who meet the eligibility requirements and complete
the Official GED Practice Test (Form PB or PC) at one of Tennessee’s Department of
Labor and Workforce Development’s recognized Adult Education programs are eligible
to receive an Official Transcript of GED Practice Test Results form. The form entitles
the examinee to register for the GED test at one of Tennessee’s GED testing centers.

The Official Transcript of GED Practice Test      Results forms are official documents
and must be kept in a secure location. Additionally, the designated notary public in the
local adult education program must affix his or her notary seal to the authorization form
before the testing center will accept the document. The authorization form is printed in
triplicate and the top two copies (white and canary) must be given to the examinee for
use when he or she registers for the official GED test. The pink copy is to be kept by
the local adult education program.

When completing the Official Transcript of GED Practice Test Results              form, the
test administrator must make certain that the name written on the form is an exact
match of the candidate’s name as it appears on his or her identification. This is
especially important because the GED test centers require candidates to register under
the same name as noted on the person’s identification. Discrepancies often arise when
an individual has recently married, divorced or has changed his or her name without
changing the name on their identification (drivers license, state issued ID card, military
ID, etc.). Supervisors may ask the test administrator to write the test candidate’s name
on the transcript form as the individual presents his or her identification at check-in.

After the test administrator completes the Test Data, the information included in the
box in the middle of the GED Transcript Form must be explained to the test candidate.
All candidates are required to sign the form on the signature line. The test candidate’s
signature indicates he or she understands the three statements included in the boxed
area. Additionally, test administrators should encourage the test candidate to release
his or her scores to their local adult education program.
               NOTARIZING THE GED PRACTICE TEST TRANSCRIPT

The lower portion of the Official Transcript of GED Practice Te st Results Form is
to be completed by the test administrator and notarized by the AE program’s
designated Notary Public.

      NOTE: The Notary process is different depending on whether the notary
      administers the practice test or if someone else administers the test. The two
      scenarios are noted below.

(1) If the designated Notary Public administers the test and completes the Official
transcript Form, he or she DOES NOT SIGN on the affiant’s signature line since a notary
cannot notarize his or her own signature. The notary does, however, complete the
remaining information on the form (Sworn to…, Notary’s signature and commission
expires). The Notary’s signature verifies the authenticity of the student’s signature.

(2) If someone other than the designated Notary Public administers the GED Practice
Test, the test administrator DOES SIGN on the affiant’s signature line. The test
administrator’s signature on this line verifies the authenticity of the student’s signature.
The Notary then completes the remaining information on the form.


          MAINTENANCE OF OFFICIAL GED PRACTICE TEST RECORDS
                       AND TRANSCRIPT FORMS

Permanent records of individuals’ Official GED Practice Test results and copies of the
accompanying paperwork (Official Transcript of GED Practice Test Results Form,
Director’s Recommendation For the GED Testing Program” and/or Form L-15) should be
maintained in a secure area according to the policies established by the Office of Adult
Education in Nashville. The Official GED Test Authorization Forms and the notary seal
must be secured and available only to designated program personnel who are approved
to sign and distribute the form.
