Please make sure you register your child AT THE SAME TIME as anyone that you want them to be sitting with. This cannot be organised afterwards, if you register separately. there are two Night rallies each evening, Monday to Friday, and eVerY rallY is DiFFereNt. as a delegate, you will automatically be allocated entry to ONe of the evening rallies each night. FUEL & WILdLIFE: Medical treatment Consent: i being the parent/guardian of the said child understand that whilst every precaution will be taken to ensure the good welfare and protection of my child, hillsong Church, its staff and volunteers acting on behalf are hereby released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to the child and their property. i hereby give permission to the First aid staff/st John ambulance to ensure proper treatment for my child. i understand that every effort will be made to contact me before instituting such procedures. i agree to pay all such doctor, ambulance and hospital fees incurred on behalf of my child. i have attached a list of any health information about my child that the First aid staff need to know. involvement Consent: i being the parent/guardian of the said child hereby give my consent that my son/daughter may participate in any activities they choose over the course of Jam, whether it is games, jumping castles, basketball, skateboarding etc. FUEL YRS 7 - 9 ONLY (AGES 12 - 15): supervision Policy: the Jam Fuel program will provide supervision at a parent pick-up point at the conference venue. No students will be allowed to leave the venue until they are picked up by a parent/guardian. it is an essential term of Jam Fuel registration that where parents will not for any reason collect their child from the supervised parent pick-up point then hillsong Church, its employees, servants and volunteers as organisers of Jam Fuel take no responsibility for any student leaving the supervised site for whatever reason. i understand and agree to be bound by this policy. WILdLIFE YRS 10 - 12 ONLY (AGES 15 - 18): student release: students attending the Jam Wildlife program will be released at the end of program without parental/guardian supervision. i understand that from the time my child leaves Jam Wildlife, hillsong Conference is not liable for anything that my child may do or any injury, loss or damage that my child may suffer.
SECTION 6 – terMs & CONDitiONs
CANCELLATIONS: a fee of $55 per person will apply to any cancellation made before 1st June 2008. No refund will be made after this date. the hillsong Conference Office must be advised in writing of any transfer, cancellation or change to your registration before 1st June 2008, subject to availability. You are able to transfer your registration only once to either another person or to the following conference year excluding registrations purchased at the Conference special rate. Please note that refunds are not available for transferred registrations. registrations purchased at the Conference special rate are only valid for hillsong 2008 and are not transferable or refundable. Your registration and payment confirm that you have accepted this policy. GROUP REGISTRATIONS: For those who have registered groups of 10 or more adults and have not provided full details of each group member, you must provide those details by 31 March 2008 or those registrations will be considered invalid. registrations purchased at the Conference special rate are only valid for hillsong 2008 and are not transferable or refundable. SEATING: Please register together to sit together. the group you register with is who you will be seated with. Once your seat has been allocated you are unable to make changes or additions to this. Please take this into consideration when registering your children. You must register together to sit together. if you require special seating (for medical conditions) for the conference please put your request in writing with supporting documentation by 1st april 2008. You are entitled to one (1) carer, family member or friend to sit with you. Please name this already registered delegate in your request. if your registration form is incomplete it will be held back and not allocated a ticketed seat until the registration has been resolved. Your registration and payment confirm that you have accepted this policy. GST: the registration price of the conference does not include Gst as the conference is considered a ‘religious service’. should you require a tax invoice for your records, please photocopy this registration form prior to returning it to us. dISCLAIMERS: the conference experience will be captured in both photographs and video. hillsong Conference reserves the right to use this material for promotional purposes. hillsong Conference recommends that you confirm your registration with our office prior to booking any flights &/or accommodation. the hiilsong Conference Office does not accept any responsibility for losses incurred for flights &/or accommodation booked for the conference. the organisers of hillsong Conference reserve the right to change any of the published conference details without notice and have the final say on any matter relating to the conference. VISAS: if you require assistance with a Visa to attend the conference, please contact the Conference Office in writing by 1st May 2008. No assistance will be given after this date. assistance will come in the form of a confirmation letter sent to you via fax and hillsong Conference will notify australian immigration on your behalf of your intention to attend hillsong Conference 2008.
