SUS/PbR Service – Registration Form 1.1 Service information The following Organisation wishes to register as a User of the SUS in the capacity of a *Sender *Recipient *Both (Please tick as appropriate) 1.2 Organisation Details Organisation Name (the User) NACS/OCS Code 0 0 EDIFACT Address Please include variants used for different data flows. DTS ID Organisation Details Address Postcode 1.3 CRG Information Ticking the copy recipient generation service box will enable the service to add additional copy recipients based on the current addressing grid and reference data. This is equivalent to the McKesson value added service but is free at the point of use. All copy recipient information on the records submitted by the provider (or generated) will be stored in the database and when online querying is enabled will be part of data access rules that determines who has the right to see data. (Sender and recipient is also part of this control) Do you wish to use the Copy Recipient Generation service? Y/N 1.4 Contact for Message Rejection We will contact the person named below when an interchange fails to be processed by the SUS Service. During the first phase, this contact will be by e-mail. Please note, this user will need to be registered on Spine and will need to provide their Spine User’s Unique Id which can be found on the my profile tab in the spine user directory. In order for this user to enable the extracts they must connect to the Ardentia product at least once through Spine. This enables a private ‘My Document’ folder to be available for the report. Where a different NACS code is used then please use an additional form. DTS ID Notes for completion Details required Organisation Name Note The name of the NHS organisation wishing to use SUS/PbR. This will be replaced in SUS/PbR with the name recorded on the Spine Directory Service (SDS) and is used here to help confirm the code is correct. The organisation code that will be used to send data to SUS/PbR. Please enter the 3 character NACS/OCS code. The EDIFACT address to be used in the data flow. The EDIFACT address is a 15 character string, the first 10 digits identifies the subscriber organisation. The last 5 digits are used to identify different sites within the organisation. The Addressing Support Officer at NHS Connecting for Health (formally NHSIA) allocates the EDIFACT address. If you require confirmation of the address contact the NHSConnecting for Health Helpdesk on 01392-251289 and log your call as an 'Access Services' query. The DTS ID for sender and recipient. Organisation codes will be transferred from the current NWCS system to SUS.
