									   Parental Permission Form for Student Information Access
Dear ProStart Parent,

Your child is part of a ten-year-old program called ProStart. ProStart was developed so high
school students could be exposed to and begin training for rewarding careers in Florida’s largest
industry – Foodservice. The Florida Restaurant Association Educational Foundation supports this
great program and is constantly working to improve it. We want to do a better job for your child
and hundreds of other deserving children. However, we need your help.

The purpose of this signed parental permission form is allow the Florida ProStart School-to-
Career Program research staff, on behalf of the Florida Restaurant Association Educational
Foundation to access ProStart student information for the purpose of improving the ProStart
Program. This information is located in the Florida Department of Education’s K-20 Education
Data Warehouse (EDW) database. We would like to gather routine student information from
your child, such as: Name, School Name, Course ID, School ID, Student ID and Date of
Birth. This information will be combined with similar information from other students around
the state to give us an overall look at how well the ProStart Program is working.

State and federal laws governing the release of student records apply to the release of data from
the EDW as well. Under no circumstances shall records and reports be released to any
party that contains individually identifiable student information. This is in strict accordance
with the provisions, and to the entities identified in, Public Law 93-80--Privacy Rights of Parents
and Students, commonly known as the “Buckley Amendment; the Family Educational Rights and
Privacy Act (FERPA), 20 U.S.C. § 1232g and 34 CFR Part 99 and the State Public Records Law,
Title X, Chapter 119, Florida Statutes, including express exceptions to Section 119.07(1), Florida
Statutes, as the same may be amended from time to time. Information from EDW such as trends
over time or the results of tracking student cohorts can be useful for the support of educational
research. The matching process and integration of data in the EDW from various sources
provides an environment that facilitates the use of data from different years as well as different
educational systems

below and return to TEACHER NAME no later than November 3, 2008. If you have any
