Completion Guide

PAGE 1: Personal Details - Please complete to give details of your personal particulars (including Driver Licence number),
including domicile and employment history. A three (3) year history is mandatory. These forms can be reproduced for additional
applicants.

PAGE 2: Asset and Liability Statement – This is to be provided to the bank with a snap shot of your current Net Asset Position.
Please complete the Income and Expenditure sections. Please include all relevant information regarding cheque or savings
accounts, regardless of size. The Fulfilment Company will then submit this information accordingly, using Macquarie’s carefully
documented guidelines.

PAGE 3: Loan Structure - Please complete this section to inform Macquarie Bank of the structure for your proposed borrowings.

PAGE 4: Account Access - Please complete this section to inform Macquarie Bank on the account access particulars including
deposit card and internet access, cash cards and the preferred means by which you would like to pay the loan application fee.

PAGE 5: Privacy Act Disclosure - This section requires the signature of all applicants to authorise the disclosure of relevant
personal information that pertains to the application. It also includes an acknowledgment that the information included in the
application is true and correct.

PAGE 6: The Acknowledgement Form - This is your authorisation that (The Fulfilment Company) are authorised to act on your
behalf as an agent of the bank, and that we are paid a commission for our services to you. This commission is fully disclosed by
us and in the bank’s letter of offer. This section also provides an opportunity to delete the clause that offers the bank to contact
you in relation to their other services.

   acting in connection with any financing provided or proposed to be provided to me/us any consumer or commercial credit information.
7. Provide information to credit reporting agencies. Give to a credit reporting agency personal or commercial information about me/us. The
   information includes identity particulars; the fact that credit has been applied for and the amount; the fact that the Manager is a current credit
   provider to you; payments which become overdue more than 60 days, and for which collection has commenced; advice that payments are
   no longer overdue; advice that cheques drawn by you have been dishonoured more than once; in specific circumstances that in the opinion
   of the Manager, you have committed a serious credit infringement; and the credit provided to you by the Manager has been paid or otherwise
   discharged.
8. Provide information for securitisation. Disclose any report or personal information about me/us to another person in connection with funding
   financial accommodation by means of an arrangement involving securitisation.
9. Provide information to guarantors. Provide information to any person who proposes to guarantee or has guaranteed repayment of any credit
