Dana College ATHLETIC SCHOLARSHIP AGREEMENT FORM Recipients of Athletic Scholarships are required to complete and return this Agreement Form before scholarships will be applied to tuition costs. Please return this with the Financial Aid Award, by the date indicated in the top, right-hand corner. I. In accepting the scholarship(s) during your enrollment at Dana College, you are declaring your intent to participate in the area(s) each year in which the scholarship is awarded. Participation includes but is not limited to: prompt attendance at all practices, meetings and performance settings and other required events as determined by the Coach; fulfillment of all assigned responsibilities; cooperation with Coach. Unless stated otherwise cumulative GPA requirements for renewal are: 1.75 after freshman and sophomore years; 2.0 thereafter. Specific participation requirements include but are not limited to: ATHLETIC SCHOLARSHIP: A. Participation includes but is not limited to: prompt attendance at all practices, meetings, and performance settings, complete cooperation with coaches, fulfillment of all assigned responsibilities and other requirements as determined by the Coach. B. Adherence to all National Association of Intercollegiate Athletics (NAIA), Great Plains Athletic Conferences (GPAC), and Dana College rules and regulations, as set forth in the NAIA and GPAC handbooks and the Dana College student handbook. C. Adherence to all team and athletic department rules as set forth by the Head Coach and the Dana College Athletic Dept. If conduct becomes inappropriate or performance becomes deficient, the Coach/Athletic Dept. reserves the right to dismiss you and revoke all or a portion of the scholarship. D. Recipients must meet NAIA and GPAC eligibility requirements prior to and during enrollment for the year. For additional information regarding eligibility requirements, contact the Athletic Director. The major requirements are outlined as follows: 1) An entering freshman student must be a graduate of an accredited high school or be accepted as a regular student in good academic standing as defined by Dana College, and meet TWO of the three following entry level requirements: a. A minimum score of 18 on the Enhanced ACT or 860 on the SAT (for tests taken on or after April 1, 1995); b. An overall high school grade point average of 2.00 or higher on a 4.00 scale; c. Graduate in the upper half of the student’s graduating class. 2) All student athletes must be “identified’ and enrolled in a minimum of 12 institutional credit hours at the time of participation. If the participation takes place in between the Interim term or during Christmas break, the student must have been identified with the institution the term immediately before the date of participation. A student is “identified’ with the institution when he or she does ONE of the following: a. Enrolls for at least nine (9) institutional credit hours b. Represents the institution in any intercollegiate athletic contest c. Attends any regular scheduled class 3) Freshmen students must have accumulated a minimum of nine (9) institutional credit hours prior to identification for the second term of attendance to be considered eligible for participation or an athletic scholarship for the second semester. 4) After completion of the second term of attendance and from then on, a student must have accumulated a minimum of 24 credit hours in the two immediately previous terms of attendance. 5) A student must be making normal progress toward a recognized baccalaureate degree and maintain the minimum grade point average as defined by the institution and the NAIA. **continued on the back** 2/2000 6) A student must meet the following GPA and credit hour requirements: a. First season: 1.50 gpa and 9 credits accumulated b. Second season: 1.75 gpa and 24 credits accumulated c. Third season: 2.00 gpa and 48 credits accumulated d. Fourth season: 2.00 gpa and 72 credits accumulated 7) If the student becomes ineligible under these requirements, the student may not continue to receive an athletic scholarship. E. Recipients agree to participate in the sport while physically able to do so. You agree to promptly notify Dana’s Athletic Trainer/Head Coach of any physical condition/injury that prohibits full participation. F. Recipients agree to notify Dana’s Athletic Trainer and Team Doctors of any pre-existing condition which may prohibit full participation. G. If an injury/illness precludes you from participation, you must notify Dana’s Athletic Trainer within 48 hours of the inability to participate. You agree to participate in proper treatment and rehabilitation as prescribed by Dana’s Athletic Trainer and Team Doctors and return to full participation as soon as possible. H. Upon verification of injury/illness, Dana’s Athletic Trainer will seek written confirmation from the Dana College Team Doctor/Orthopedic Surgeon. Upon release from participation you agree to provide assistance to the Athletic Department (under the supervision of the Athletic Director) in lieu of the scholarship. I. Recipients understand that only unexpected injury/illnesses that are beyond the control of, or occur as a direct result of, participation in said sport allows for scholarship renewal in the year the injury occurred. If athletes are unable to return to full participation in subsequent years, the Coach/Athletic Dept. reserve the right to reduce or revoke a player’s scholarship. J. Normal academic progress and regular classroom attendance is expected for the duration of the scholarship. K. Athletic scholarships are available only during years in which athletes are eligible and participating, and are not guaranteed during redshirt and/or years after athlete has used their eligibility for participation. Exceptions to this are outlined in letter A-J above. II. You agree to represent the team and Dana College in a positive manner. If your conduct become inappropriate, the Coach reserves the right to dismiss you and revoke all or a portion of the scholarship. III. Regular classroom attendance is expected, and full-time status must be maintained for the duration of the scholarship period. You understand that by accepting the award it is your responsibility to provide prompt notification to the Coach if you choose to discontinue participation. If you become ineligible to or do not participate in the scholarship area(s) in the manner described above, you will forfeit your right to that scholarship. The College may revoke any aid previously awarded for that area, and will determine your eligibility for other available financial aid. It will be the latest dated Financial Aid Award which will be used to determine the sum owed the College for the costs associated with the semester or year you are enrolled.
