EVALUATION
The programs in this section facilitate the employee evaluation process.

Evaluation Forms

Evaluation forms within HRMS are for included for your convenience. To use this form, you must print
them for distribution to the persons involved in the interview process. No on-line data entry is allowed.
The following forms are available:

       FODI
       Pre-Conference Questions
       Pre-Conference Form
       Pre-Conference Scoring Rubric

When you select one of the evaluation forms, the system opens a window displaying blank form. You
may print the forms using the Print option within the File menu, located on the menu bar at the top of your
screen.

Viewing an Employee’s Current Evaluation Status

Enter a social security number to display an employee’s current evaluation status. The system will display
current evaluation information for the selected employee. To enter an evaluation for the current year,
click on the submit button at the bottom of the screen.

Evaluation System Views

View evaluation that has been conducted: Click on this text to view a list of employees who have
evaluations for the current year.

View Employee(s) that still need Evaluation(s): Click on this text to view a list of employees who do not
have evaluations for the current year.

Completing the Employee Evaluation Screen


Section 1: Basic Information

The information in this section was entered when the employee was hired and assigned to a position. No
data entry is allowed. To update the information in this section, use the appropriate program within the
Staff Action section. The following information is displayed:

       Employee Status
       Primary Position
       Site
       Title
       Description
       Other position

Section 2: Evaluations For

Select year from the drop down list box. Use this option to view past employee evaluations. The system
will return evaluation information for the selected year.

Section 3: New Evaluation

Use this section to create a new evaluation for the current year.

Select School year: Select year evaluation is to be conducted from drop down list box

On Year: Select Yes or No.

Choose Evaluation Instrument: Select FODA or Summative from the drop down list box
Click on Submit at the bottom of the screen to create a new evaluation form. The system opens another
window in which the blank form is displayed.

FODA or Summative: When the evaluations form appears, complete all required fields as indicated by *,
as well as evaluation ratings and comments. You may use a word processing program to and copy and
paste comments if desired. Select Submit and the successfully updated screen will appear.
