To be Submitted to Division Chair when an Incomplete is Assigned
Name of Student _______________________________________________________________
Banner ID Number _____________________________________________________________
Course and Number _________________________________________ CRN ______________
Term Taken ________________________________________________ Year ______________
Justification of Assignment of Incomplete:
Division Chair’s Signature
If a student is unable to complete a course because of circumstances clearly beyond his/her control,
the instructor may assign an “I” for that course by completing the Incomplete Grade Assignment
Sheet form and submitting it to the Division Chair for approval, after which it will be forwarded to
the Registrar’s Office. The instructor will then assign the student an “I” on the final grade sheet.
The student will have one academic year to complete the required work. (Ex: “I” received Fall 2001
must be made up by the end of Fall 2002) Incompletes are made up by working independently with
the instructor and not by registering for the same course again. When the missing work has been
satisfied, the instructor will submit a Grade Change form through the Division Chair and the
Academic Dean to the Registrar. If the “I” has not been completed within the academic year, the
“I” will automatically revert to an “F”.
An “I” cannot be changed to a “W.” Only grades of “A,” “B,” “C,” “D,” or “F” can be assigned
The Official Grade Change Form must be received in the Registrar’s Office by the official
“Grades Due” date for the term the requirements must be completed.
