COST: The 2 day seminar is $299 per person. ($149 for each additional attendee from the same company)

money order or bank check mail payments, along with this signed registration form, to BOLDStone Conference Center, Attn. Amy Gudnason,
4261 Sunset Lane, Suite D, Shingle Springs, CA 95682. Please make checks payable to BOLDStone. Include the event number (B032910) and
attendee’s name(s) on all checks. Attendance will be limited, so please register early.

CANCELLATION/REFUND POLICY
To cancel your registration, contact us at least two working days before the seminar date for a refund, minus a $100 cancellation fee, or receive
a full credit toward the price of any seminar scheduled during the following 12 months. Without advance notice, we cannot issue refunds or
credits, but you are always welcome to send a substitute from your organization to the seminar.

SEMINAR CONFIRMATIONS
Your seminar registration will be confirmed via e-mail (only) if it is received more than 5 working days before the seminar. To receive a
confirmation, provide the appropriate e-mail address(es) on this registration form. Please note that some confirmations do not make it
through various spam blocks, firewalls, spyware, and other system protections.

SEMINAR SCHEDULE DISCLAIMER
We reserve the right to make changes to our seminar schedule at any time. These changes may include, but are not limited to, additions,
cancellations, and/or location changes. Every attempt will be made to keep registrants apprised of any changes that might affect their ability to
attend.

$ PayPal or Credit Card payments: If this is the option you want, BOLDStone will email you an invoice with a link to PayPal. If you already
have an account set up with PayPal then BOLDStone must use the email you use that is associated with that PayPal account. If you do not
have an account with PayPal BOLDStone would like to know what email address would you like the invoice and link to PayPal to be sent too?
