					An exit interview is an interview conducted by an employer of a departing employee.
The purpose of an exit interview is to gather the employee's feedback on the work
experience in order to improve working conditions and employee retention. This
document provides a system of checkboxes to gauge an employee’s satisfaction as to
certain work related issues, including satisfaction with leadership, with work content,
and with the company. This form can easily be customized to add or alter any
questions an employer would like to ask a departing employee.
