SelectSurvey.NET Quick Start Guide
1. Click on this link, or type the following URL into
2. From the main screen click on the word “here”
within this text.
3. Type in a username and password; these are the same
as the KSD username and password.
4. This log in process will allow users to take surveys.
The Survey ID is required. In the future, all KSD
employees will be able to create surveys just by
logging in. If you need to create a survey and are
unable to at this time, please call the CSC at 7030.
From this page, options will then be:
Manage surveys (create surveys)
Edit your account

From the Manage Surveys page, many options are available.
Surveys - Manage surveys
Libraries – Manage question libraries for often used questions
Email Lists – Create email lists for survey invitations or create
customized email invitations
Reports – To customize results when surveys are completed
Edit Existing Surveys
Page Help Design – Manage features and build your survey questions
Options – Choose various options like start and end dates and
respondent access levels
Deploy – Launch your survey by choosing an email link, weblink
or pop-up survey link
Create New Analyze –View survey results
Survey Note: Each screen has a page help button that guides you through
the options available.
This area shows Existing surveys that you own or have rights to manage.

Create New Survey (Design/Modify Survey)
1. Click on the Create New Survey button to create a new survey.
2. Enter a name for the survey and click Save. A default template is used with the KSD logo.
3. Use the Insert button to create Question/Item types (see below for descriptions)

4. Continue to Insert and/or
Edit question/item types.
5. Move them on the survey
as desired by using the
up and down arrows on
the left of each question
6. Multiple pages can be
created if needed and
question/item types can
be moved between pages
with the Move button.
7. Click the Done button at
the bottom of the page when the survey is completed.
8. Back on the Manage Surveys page; select Design at any time to re-edit the survey.
9. To view the survey as users would, click on the title of the survey.
10. Select Options for management of survey as desired.
11. Select Open to allow others to take the survey.
12. Select Deployment to send the survey to users.
13. For survey results, select Analyze.
Note: When creating a survey there are various Question/Item types:
o Database dropdown – This type of question is only used if the answers are choices that
will come from a database query.
o Multiple Answers (checkboxes) – Multiple choice questions with checkboxes. You can
choose vertical or horizontal check boxes and a minimum (1 answer only) or a maximum
number of answers. You may choose from pre-defined answers in the application or create
your own answers. You may also assign a point value for each answer (for testing).
o One Answer (Dropdown) – This is for a one answer only question and the answer is
chosen from a dropdown list. You may choose pre-defined answers or create your own.
o One Answer (Option Buttons) – This is for one answer only and the answer is chosen
from buttons either vertical or horizontal (similar to the checkbox).
o True/False – You may choose from a dropdown, buttons horizontal or buttons vertical.
o Yes/No – Same as True/False above.
o Multiple Answers per Row (Checkboxes) – Use this for survey questions that have more
than one category or sections that require answers.
o Multiple Answers per Row (Textboxes) – Same as above except textboxes are used for the
answers instead of checkboxes.

o One Answer per Row (Option Buttons) – Same as above except only one answer allowed
o Rating Scale (Numeric) – Same as above except each row has a numeric rating scale i.e.,
o Comments Box – you may customize the width of the comments box.
o Constant Sum – You may require an answer total or allow any total.
o Date – You supply the earliest date possible and the latest date possible. Format for the
date is mm/dd/yyyy.
o Number – You supply the minimum and maximum values as well as the maximum
allowable number of characters.
o One line – You supply the maximum number of characters and textbox width.
o One or more lines – Same as above except multiple lines.
o Ranking – Same as above but used for ranking scale type questions.
o User Registration – Used for user registration to be stored in a database. This would only
be used if the survey creator required user registration.
Presentation (Design items)
o Heading – Inserts a heading into your survey.
o Horizontal rule – Inserts a horizontal rule element.
o HTML – Insert any HTML and/or content.
o Image – Insert an image path to insert an image in to you survey.
o Message – Insert any message into your survey.
Note: All questions (when applicable) contain these options:
1. Question alias for alternate text that will show on the report.
2. Sub-text for additional information relating to the question (the survey taker sees this text).
3. Required for users to answer.
4. Randomize answers so they are in different locations in each survey.
5. Allow additional answer or a text box for additional comments.
Manage Survey Libraries
Questions can be reused by other surveys and are managed through Libraries. This is especially
helpful for questions used repeatedly.
Manage Email Lists

Use this page to create an email list of people to take your survey. The email message and link to the
survey can be customized here.
If you have a group of individuals that you consistently send surveys to, you can use the email list
function to save time when deploying your survey. Simply import or paste a list of email
recipients in to the text box provided on the “import email list” screen. Please view the page
