3. Does any tattoo, brand, and/or body ornamentation visible while wearing standard issue Navy PT
   gear exceed the area of the wearer’s hand, with fingers extended and joined with the thumb
   touching the base of the index finger?
4. Does any tattoo, brand, and/or body ornamentation coverage exceed one-quarter of the respective
   body part?
5. Has the applicant ever had any tattoo, brand, and/or body ornamentation removed?

NOTE: IF THE APPLICANT HAS RESPONDED “YES” TO ANY OF THE ABOVE QUESTIONS, THEY SHALL BE INTERVIEWED
BY THE COMMANDING OFFICER OR EXECUTIVE OFFICER TO DETERMINE IF THE APPLICANT IS ELIGIBLE FOR A
WAIVER FROM THE APPROPRIATE CNRC OFFICER PROGRAMS DIRECTOR. APPROPRIATE DIGITAL PHOTOGRAPHS OF
BODY MARKINGS SHALL ACCOMPANY THIS FORM WHEN REQUIRED. IF YES TO QUESTIONS 1 THROUGH 5 ABOVE, A
TATTOO SCREENING CERTIFICATE SHALL BE FORWARDED WITH PRODOCS TO COMNAVCRUITCOM.

   discredit upon the Navy?
8. Are any of the tattoos a result of a specific activity? (i.e., specifically an illegal activity or as the
    result of any violation of law(s))?

Any “Yes” response to item 6, 7, or 8 above is disqualifying, no waiver is authorized.
