This is the form for the month of ___________. Please use this form to help you
prepare your Book Report and submit this form on the day of your book report
presentation for evaluation.
Your Book Report must include the following:
1. Title page that includes:
- The title of your book
- Author’s name
- Your name
- Due date
- A hand drawn picture that represents your book
2. Setting: write a paragraph describing the setting (place (country, province,
city building), time, time of the year, era etc.
3. Plot. Briefly summarize the plot for us (no more than half a page). Outline
the conflict and how it was resolved.
4. Character: write a paragraph about a character in the story. Describe the
character’s: age, appearance, personality traits etc. Tell if you liked this
character why or why not. Use a piece of evidence (a quote) from the text
to help explain your character.
5. Graphic Organizer: create a graphic organizer for your novel (i.e., story web,
Venn Diagram etc.). See Writer’s Express pages 296 – 297 for more
examples.
6. Spelling, grammar and punctuation are important part of this assignment.
Be sure to carefully edit and revise your report.
