							            International Programs Annual Photo Contest Entry Form and Waiver

              Please complete and attach these forms to each photo submission

UI students that have studied abroad and UI international students may submit up to 2 photos
by October 20, 2009. Please submit your photos in both 8”x 10” inch print format and high-
resolution electronic format. The prints, this form, and the waiver may be submitted to 1111
University Capitol Centre, Office for Study Abroad. Electronic submissions may be sent to study-
abroad@uiowa.edu.

We require that you submit an 8” x 10” print in addition to your electronic submission, as the
print quality of electronic submissions cannot be guaranteed. Photos may be cropped and
treated for color enhancement, but should not possess extensive photo shopping (e.g. adding
elements from different photos into one shot). Selected photos will be displayed in the IMU
during the month of November. A reception will be held in the Old Capitol on Thursday,
November 19th during International Education Week (November 16th through the 20th).

I give permission for my photo to be used in future International Programs literature and
publicity. I have also signed and attached a Waiver and Release Form in case my photo is
selected for display in the IMU gallery space.

                                    Waiver and Release Form

        In consideration for being allowed to participate in the University of Iowa International
Programs Annual Photo Contest, co-sponsored the Office for Study Abroad and the IMU Gallery,
I,______________________________, by signing my name below, acting for myself, my heirs,
personal representatives and assigns, do hereby release, waive and forever discharge The
University of Iowa, the State Board of Regents, the State of Iowa and their employees, agents
and representatives from any and all liability of any and every nature whatsoever, including
claims or suits at law or in equity, that that I may have for any and all property loss or damage
which may result from my participation in such event. I agree to the reproduction of my artwork
for promotional purposes and I guarantee that there has been no violation or infringement of
copyright laws for any of the artwork shown in my exhibit.

        By signing below, I understand the potential for damage or loss of my property during
the exhibition to be held at a campus location during the entire month of November including
while transporting to and from this site. The risks include, but are not all-inclusive: artwork could
be broken partially or totally or stolen.

         In signing this release, I acknowledge and represent that I have read the foregoing,
understand it, and sign it voluntarily as my own free act and deed; no oral representations,
statements, or inducements, apart from the foregoing written agreement, have been made; and
I am at least eighteen (18) years of age and fully competent.
