          2009 (Calendar Year) Nominations Form Guidelines

Please note the on-line form will be available December 21, 2009.
Please review these guidelines carefully and use them to prepare materials,
prior to accessing the on-line form.

General Information:
The Award categories, criteria and processes have been revised by the
Awards Committee.

Prior to completing the on-line nomination form, please review these
submission guidelines carefully and prepare your materials for your
nomination submission.

Once the nomination is submitted, the nomination information that you
provide is considered “FINAL” and cannot be revised or altered.

      Mark Your Calendar - Important Dates:

 December 21, 2009
The on-line nomination forms for each of the Award for Excellence Categories will be
posted on the OACCAC Website.

 February 12, 2010
On-line nomination will be closed. All nominations will be verified with an e-mail from
Antian Professional Services Inc. If you have not received a verification e-mail from Antian
Professional Services Inc. by February 12, 2010, please contact:

 March 19, 2010
The OACCAC will not take formal Nominee Photographs at the Nominee Reception. Informal
photographs will be taken at the Nominee Reception. In order for nominee photographs to be
included in the Awards Video presentation that will be used at the AWARDS BANQUET,
JPEG Photos must be submitted to Antian Professional Services Inc. by March 19, 2010.

                      Award Criteria and Submission Guidelines
Please review these guidelines prior to filling out the appropriate on-line form below depending
on the award category your CCAC is entering.

Notes:
    A separate on-line form must be submitted with each entry
    Each CCAC is allowed only one entry per category
    Each CCAC is allowed up to 250 words under each criterion
    All entries must be submitted electronically by February 12, 2010 to Antian
       Professional Services Inc. using the on-line form that will be posted on the OACCAC
       website on December 21, 2009.
    Submitted nominations that do not fit within the identified criteria for that category will
       not be evaluated and the submitting CCAC will be asked to provide clarification, within
       an identified time frame, early in the evaluation process.

DETAILS:

CCACs are allowed one nomination entry to each of the five award categories
below:

   1.   Individual CCAC Staff
   2.   CCAC Staff Team
   3.   Leadership
   4.   Citizenship
   5.   Systems Partnership – with other CCAC(s) or outside organization

By submitting the nomination form, the Nominee(s) and CCAC agree to the
nomination and publication of the submitted information by the OACCAC.

Reminders
   A separate on-line form must be submitted with each entry and completed in
     full.
   Each CCAC is allowed only one entry per category.
   The submission form is designed to provide limited space (250 word limit
     per criteria).
   All entries must be submitted by February 12, 2010.

General Eligibility Criteria - Nominees for OACCAC Awards of Excellence
must:

   1.   reside or work in Ontario
   2.   be in good standing with the CCAC/OACCAC
   3.   be associated with the CCAC/OACCAC organization
   4.   agree to the nomination before information is submitted to the
        OACCAC Awards Committee

Nominations for the awards will be accepted only through the CCACs.
Support from the CEO and/or Board Chair is required to validate the achievements.
Previously unsuccessful submissions may be re-submitted.
The CCAC will support its nominee(s) through the nomination, selection and
recognition process.

Such support may include but not be limited to:
         preparation and submission of nomination materials by February 12,
            2010
         the provision of photos of the nominee(s) as well as photos of
            innovative projects, completed pieces of work or special events that
            substantiate the nomination must be submitted by March 19, 2010
         financial assistance when possible to send the nominee(s) to the
            OACCAC Conference Awards Banquet, June 14, 2010.

I.      Individual CCAC Staff Award Criteria

        This award acknowledges the efforts and commitment of a CCAC
        employee who has demonstrated excellence and caring in contributing to
        the CCAC goals and objectives
Background – (Optional) can be provided up to 250 words.

Please outline the individual CCAC staff’s accomplishments beyond their ongoing job
responsibility that demonstrate the following:

      Trusted Quality Care Connector:
       set performance and customer service quality standards for the
          delivery of care
       create connections for people by developing, coordinating and
          maintaining links across the health care services system

   2. Exceptional collaboration with co-workers

     3. Engagement with providers, local professionals, organizations,
        etc., to enhance quality client care
     4. Demonstration of informal leadership
     5. Passionate commitment to quality and best practices

   CCAC Staff Team Award Criteria

           This award acknowledges the efforts of a CCAC staff team that has
           demonstrated excellence in contributing to the CCAC/OACCAC goals
           and objectives
Background – (Optional) can be provided up to 250 words.

Please outline the accomplishments of the staff team nomination to demonstrate the following:

   1. Outstanding collaboration within or across portfolios in the
      CCAC including sector initiatives such as CHRIS
      implementation, Branding initiatives, Service Guarantee
      initiatives, etc.
   2. Positive partnerships with providers, local professionals,
      organizations or CCACs that focus on client care
   3. Embody leadership and inspiration through high-functioning and
      effective team work
   4. System-wide outcomes that enhance a positive client experience
      e.g. Care Connector, Branding initiatives, Service Guarantee
      initiatives, etc.
   5. Innovative ideas or approaches that demonstrate a commitment to
      continuous learning

As the judges will have varying exposure to your local issues, please ensure that the impact of
the above achievements is evident in your submission. Responses should be concise and listed

   CCAC Leadership Award Criteria

       This award is to commend a board member or a staff member, excluding
       CEOs, who has demonstrated outstanding leadership qualities in the
       CCAC/OACCAC environment.
Background – (Optional) can be provided up to 250 words.

Please outline the accomplishments of your CCAC Leadership nominee in demonstrating the
following:

   1. Leadership qualities that are inspirational and exemplify the
      CCAC vision, mission and values
   2. Local, regional or provincial collaboration to develop positive,
      productive relationships with stakeholders including other
      CCACs and the OACCAC

   4. Development of strategic initiatives to enhance the CCAC role as
      a trusted quality care connector
   5. Demonstrate a level of involvement and positive effect on the
      CCAC

IV.    CCAC Citizenship Award Criteria

       This award is to commend a CCAC/OACCAC board or staff member who,
       through volunteer work, community involvement or other extra-curricular
       activities, has made a significant contribution to a community or group.
Background – (Optional) can be provided up to 250 words.

Please outline the accomplishments of your CCAC Citizenship nominee in demonstrating the
following:

   1. Sincere commitment to make a difference in a community,
      organization or cause through volunteer work, community
      involvement or extra curricular activities
   2. Responsible mentorship, leadership and/or being an active,
      effective and dedicated participant

      or cause served
   5. Exemplifies a level of engagement that demonstrates a continued
      collaborative relationship with communities/organizations

V.     Systems Partnership Award Criteria

       This award recognizes the efforts of collaboration with system partners
       who are engaged in a one-time or extended project on a local, provincial
       or national level including working with another CCAC, other agencies or
       a combination of more than one CCAC working with other agencies.
Background – (Optional) can be provided up to 250 words.

Please outline the accomplishments of your CCAC with other CCACs/community partners that
demonstrate the following:

Preparation Checklist for on-line nomination entry                               Completed
 1. The significance and scope of the project
 2. Impact of the partnership collaboration on the immediate
    community, the local CCAC and/or the provincial collective CCAC
    group
 3. Positive project outcomes or innovations impacting on the welfare of
    CCAC clients and the health care system
 4. The evolution of the partnership and the resulting synergies
 5. Important systemic improvements made as a result of the partnership
    contributions

Please ensure the names and additional information are correctly spelled and
accurate, as this information is considered “FINAL” once submitted on the

   1. List last names alphabetically and indicate the appropriate job title of each person.
   2. In the case of a nomination for the Systems Partnership category, please identify the
      CCAC or external organization that the nominee belongs to next to the person’s position
      in the role section.
   3. If possible please identify who will be attending the Awards banquet, as well as who
      will be accepting the award if the nomination is successful.

Please ensure accuracy as names will appear on all award certificates as listed on this
nomination. Names will not be added nor deleted once the nomination has been submitted.
