Form of Registration Certificates of Health Care Professionals Regulation No. 145 of the Minister of Social Affairs of 21 December 2001 (RTL1 2002, 2, 22), entered into force 7 January 2002. This Regulation is established on the basis of subsection 31 (3) of the Health Services Organisation Act (RT2 I 2001, 50, 284). § 1. This Regulation establishes the form of registration certificates of health care professionals for which a standard form is annexed hereto. § 2. The registration certificate of a health care professional is an administrative document of the Health Care Board, which is prepared and printed in A4 format on a form with security features and a printed guilloche pattern background and for which the requirements for the requisite information are the following: 1) in the letter heading of the form the word “TERVISHOIUAMET” [Health Care Board] is printed in capital letters in the centre and the image of the small national coat of arms is printed above it; 2) the name of the administrative document “REGISTREERIMISTÕEND” [Registration Certificate] and the number of the certificate are printed in capital letters below each other in the centre; 3) the layout of text and the size of print on the form shall allow the whole text to be printed on the same page; 4) upon preparation of the registration certificate, information shall be entered on the form either by using a computer or typewriter in the size and font of print established by the Health Care Board. § 3. The Health Care Board shall issue registration certificates to health care professionals according to all the requirements for requisite information set out in the annex and a competent official of the state register of health care professionals maintained in the Health Care Board shall sign the certificates and provide these with numbers and dates. § 4. The Health Care Board shall establish the design of the forms of registration certificates according to the requirements, the choice of paper and the security features and guilloche pattern background thereof and shall organise the preparation of the certificates. § 5. The Health Care Board shall be responsible for the standard layout of the requisite information on the printed forms and the durability of the paper of the forms and for the security features being identical in order to ensure the unforgeability of registration certificates.
