This form must be submitted to the Alabama Department of Education. A cashier’s check, money
order, or receipt for online payment (https://www.alabamainteractive.org/education/) for the
non-refundable $20 application fee must be included. Fingerprint cards, separate fingerprint fee,
and related forms must also be included.

This form must be sent to current or former employing superintendents.
□ Fingerprint Cards (only if prints are not on file for both ABI and FBI)
Fingerprint cards cannot be posted online. You may request fingerprint cards by sending an
email to tcert@alsde.edu or calling 334-353-8567. The cards must be submitted with a one-time,
non-refundable $49 fee in the form of a cashier’s check, money order, or verification of online
payment

and FBI)

□ Supplement RTD—Retired Teachers or Instructional Support Personnel Only
An applicant must have retired from a teaching or instructional support position (principal, counselor, librarian,
etc.) in an Alabama public school system and must have had at least 25 full years of satisfactory educational
experience and a valid Alabama Professional Educator Certificate at the time of retirement. Also see
