Proposals are being solicited for the approximate service period of September 1st, 2004 through June 30th, 2005. Those interested in providing services may receive proposal packages by contacting Karl Cetina, Medina County Drug Abuse Commission, 246 Northland Drive, Medina, Ohio, 44256. All proposals shall be filed in a sealed envelope marked “Medina County Youth Tobacco Prevention and Cessation Project” and must be received in the Office of The Medina County Board of Commissioners; 144 North Broadway, Room 201, Medina, Ohio 44256 no later than 1:30 p.m., Ohio time, Friday, August 6th, 2004. To be advertised: 7/9/04 Medina Gazette Request For Proposal Medina County Youth Tobacco Prevention and Cessation Project The Medina County Board of Commissioners (hereinafter “County”) and the Medina County Drug Abuse Commission (hereinafter MCDAC) are requesting proposal submissions from interested parties to serve in the capacity of Independent Contractor for a Medina County Youth Tobacco Prevention and Cessation Project (hereinafter “Youth Project”). It is estimated that services will be required for a period of approximately ten (10) months, commencing on or about September 1st, 2004. The County will be the contracting authority. It is currently envisioned that the Youth Project will consist of providing tobacco prevention and cessation services for youth K-12 throughout Medina County utilizing currently recognized research-based programs and applying appropriate scientific methodology in compiling the raw data and subsequently producing outcome data per Ohio’s Tobacco Use Prevention and Control Foundations expectations. Objectives of Youth Project The Medina County Board of Commissioners was recently awarded an Implementation Grant from The Tobacco Use Prevention and Control Foundation (TUPCF) for the purpose reducing the use of tobacco products within Medina County. MCDAC will spearhead this effort by managing the grant, providing the project director and tobacco coordinator. Upon successful completion and analysis of the Implementation year data, MCDAC will work with the Medina County Tobacco Information and Prevention (MCTIP) coalition in writing the second year TUPCF Implementation Grant with multiple focus areas based upon the first year’s results. Contents of Youth Project • • Project responsibilities defined. Implement countywide youth tobacco prevention and cessation services. 1. Tobacco Awareness Program (TAP) and Tobacco Education Group (TEG) both utilized for Youth Cessation. 2. Stay Tobacco Free Athlete Mentor Program (STAMP) implemented for Youth Prevention. 3. Spearhead expansion effort of County Kick Butts Program. 4. Assist with development of Medina County Youth Stand Campaign. Report progress in monthly MCTIP Coalition Meetings. Strategize plans in MCTIP Youth Subcommittee. Compile database, analyze data and report conclusions to MCTIP Coalition. Identify service gaps and make recommendations to the MCTIP Coalition. Interim and Final report to be completed and presented to MCTIP Project Director. Assist Coalition in Submission of TUPCF Implementation Grant Year Two. • • • • • • Description of Youth Project Implementation of youth tobacco prevention and cessation programs, services and resources throughout Medina County. Subsequently showing positive outcome results by decreasing the utilization rates of tobacco products by Medina County youth based on thorough review of the specific data gathered via the data collection process. Management of the Project The Youth Project will be done under the direction of the County and MCTIP. The contractor will be required to provide project updates when requested. Selection Criteria The following criteria will be utilized in the selection process: • • • • Quality of proposal and specific approach to this project. Ability to meet or exceed the requirements of this RFP. Credentials of personnel. Consultant shall identify the specific individual(s) who will be in charge of the project and provide their Performa. Cost of contract to complete the work required shall be specified. SAMPLE CONTRACT AGREEMENT This AGREEMENT, made this ________ day of August, 2004, by and between the Medina County Board of Commissioners, (hereinafter called “Board”), and _________________________________________(hereinafter called “Contractor”). WITNESSETH: That for and in consideration of the payment and agreements hereinafter mentioned: The Contractor will commence and undertake the delivery of specified contract services for The Medina County Youth Tobacco Prevention and Cessation Project. The Contractor will furnish all staff, materials, supplies, and labor necessary for the successful delivery of said services. The Contractor will commence services required by the CONTRACT DOCUMENTS no later than September 1st 2004 and shall complete services by June 30th, 2005. The Contractor agrees to perform all of the DUTIES described in the CONTRACT DOCUMENTS and agrees to comply with all the terms therein for the total contract sum of $________________.00. The term “CONTRACT DOCUMENTS” means and includes the following: A. Request for Proposals B. Program Specifications C. Instructions to Proposers D. Proposal E. Agreement This AGREEMENT shall be binding upon all parties hereto and their respective heirs, Executors, administrators, successors, and assigns.
