Urban Aboriginal Diabetes Community Workshop Program The Healing Trail A Passageway to Diabetes Prevention and Control Program Priorities  To provide a community based diabetes prevention and awareness program through education and awareness about the seriousness of diabetes and the importance of prevention and control of the disease To provide culturally appropriate materials To respond to the increasing need for informed and accurate information and resources This program was developed by the Healing Trail, an Urban Aboriginal diabetes prevention and awareness program, funded by the Aboriginal Diabetes Initiative “MOAUIPP”   Funded by Health Canada: Community Responsibility  Prevention – Development of Culturally appropriate programs and materials  Education – Educating lay educators, community leaders and communities at the grass roots level  Care and Support – Ensure local community access to diabetes health promotion programs and available resources The Program Is Designed For:  Community leaders, health outreach workers or lay educators who have been trained in diabetes prevention and awareness to deliver services at the grass roots level. The Program will enable you to:  To deliver accurate information regarding diabetes in a workshop format To prompt discussion and to encourage group interaction  Quick Reference - Presentation Summaries  Community Responsibility – Provides statistics about diabetes amongst Aboriginal people and the Healing Trail mission. It describes the Healing Trail’s role in the facilitating of communities taking ownership of the disease and developing a community diabetes health care team.  Defining Diabetes – It explains what diabetes is and reviews the signs and symptoms. It outlines the summary of its risk factors and introduces key aspects of the management of diabetes. Quick Reference - Presentation Summaries  Management of Diabetes – It summarizes the need for individuals with diabetes to balance food, physical activity and medications. The goal is to present the need to manage blood sugar levels within the normal range.  Long-term Complications & Diabetes – It defines the long-term complications associated with diabetes;self-care practices, children with diabetes and ways to prevent complications. Quick Reference - Presentation Summaries  Stress & Diabetes – Reviews the relationship between stress and its impact on diabetes. It outlines sources of stress and strategies for how to cope with stress.  Benefits of Physical Activity – Explains how physical inactivity impacts all the risk factors for diabetes. It discusses the benefits of being active, physical activity guidelines, tips on how to get active and strategies to stay active. Quick Reference - Presentation Summaries  Healthy Eating – Identifies the components of a healthy diet; an overview of the Good Healthy Eating Guide, an explanation of carbohydrates and their impact on diabetes.
