(3) Summarize the proposed research, including scientific goals and experimental approach (use <3000 characters):

(4) If this is a supplement to a previous request, provide the date of this previous request and describe the progress
of the study, the need for additional samples, and the status of the previous samples (use <3000 characters):
SAFOD Sample Request Form p. 2

(5) List the types of data you will generate (for example, major/minor elemental analyses, XRD spectra, SEM/TEM

(6) Are you requesting (please check all that apply)?

Spot core samples (Phases 1 and 2)
        Spot core samples (Phase 3)
        Washed & dried drill cuttings
        Unwashed drill cuttings, bagged wet
Unwashed drill cuttings, bottled in drilling mud
Drilling Mud Samples, bottled (without cuttings)
To describe, photograph or analyze cores or cuttings at Gulf Coast Repository
Formation fluid samples (gas or liquid)
        Thin sections
        Cores or cuttings for display
        Other:

Choose one of the following two sampling options. CHECK ONE.
It is ok to move core sample locations by up to 10 cm.
It is not ok to move core sample locations.

(7) Provide general information on special sample preparation or handling procedures required, to supplement
sample-specific instructions you provide under Item 9, below. Please note that subsampling by the staff of the IODP
Gulf Coast Repository (GCR) on core samples is currently simply limited to cutting of slabs and mini-cores using
either deionized water, brine or air as a lubricating fluid. Thus, for unusual sampling requests, you may be required
to either assist in subsampling at the GCR or provide additional funding and/or equipment to the GCR. Please note
that in order to best preserve the Phase 3 SAFOD core collection, all subsampling of SAFOD cores will be
performed by GCR staff or by visiting scientists under the supervision and assistance of GCR staff; i.e., bulk core
samples will not be sent to your institution for further subcore, slicing, etc.

(8) State what condition the samples will be in when they are returned to the Gulf Coast Repository. This
information will be used to decide what tests or analyses these samples may be suitable for when you are done with
them, and should include the mechanical condition of the samples (intact, mechanically strained/fractured,
dissagregated, etc.) as well as the chemical condition of the samples (fluids used in sample preparation and testing,
leaching/etching procedures used, etc.).

 (9) SAMPLE REQUESTS:

PHASE 1 AND 2 SPOT CORE SAMPLES: Provide the following information (where available) for each sample
requested. Depths are measured depths (MD) relative to the drill rig floor as determined during coring operations.

IMPORTANT NOTE: All requests for Phase 1 or 2 core samples must be accompanied by an annotated version of
the corresponding high-resolution core photographs (downloadable from the SAFOD web site), on which you have
superimposed circles, rectangles or dots directly on the core photographs to depict as accurately as possible the
location and size of each sample you are requesting. The image file name should include the last name of
 Investigator 1 on p.1 of this form (e.g., Smith_Samples.jpg).

Depth Box Run Section Piece Sub-piece Dimensions (cm), if solid, Special Instructions (e.g.,
(ft, MD) or Volume (cm3), if rubble sample orientation, preparation
                                                                                   procedures)

PHASE 3 SPOT CORE SAMPLES: Provide the following information for each core sample requested. Depths
are measured depths (MD) relative to the drill rig floor as determined during coring operations.

IMPORTANT NOTE: All requests for Phase 3 core samples must be accompanied by an annotated version of the
Phase 3 Core Atlas (in PowerPoint format, downloadable from the SAFOD web site), in which you have
superimposed circles, rectangles or dots directly on the core photographs to depict as accurately as possible the
location and size of each sample you are requesting. To keep file size to a minimum, please delete all non-
annotated PowerPoint slides from the Core Atlas before returning. The PowerPoint file name should include the
last name of Investigator 1 on p.1 of this form (e.g., Smith_Samples.ppt).

Depth Hole Run Section Dimensions (cm), if solid, Special Instructions (e.g., sample orientation,
(ft, MD) (E/G) or Volume (cm3), if rubble preparation procedures)

THIN SECTIONS: As shown on the ICDP web site for SAFOD, a number of thin sections were prepared by the
SAFOD Project from cuttings and core samples from all three SAFOD Phases and are available for loan to
interested investigators for non-destructive examination. Please specify the thin sections you are interested in
borrowing and indicate what these sections will be used for (optical examination, SEM/probe work, etc.). Please
note that polished thin sections (suitable for probing) are also available for loan from the side-wall cores
collected during Phase 2 (see ICDP web site for descriptions and photographs of these cores).

CUTTINGS SAMPLES AND MUD SAMPLES: Please specify the depths and approximate quantities of drill
cuttings and drilling mud you are requesting from the following categories:

Washed & dried drill cuttings, collected routinely during Phases 1 and 2 at the following intervals: Small samples
(~30 in3) every 10 ft, large samples (~250 in3) every 100 ft and extra-large samples (~1500 in3) every 300 ft. Large
bags of washed cuttings were also collected before, during and after major drilling breaks (potential faults) in Phase
2 and in the rotary drilled portions of Phase 3. See SAFOD web site for depths at which these special cuttings
samples were collected.

Unwashed drill cuttings, small samples (~30 in3) bagged wet every 10 ft during Phases 1 and 2

Unwashed drill cuttings in drilling mud, collected in 250 ml bottles every 100 ft during Phases 1 and 2

Drilling mud in bottles (without cuttings). See SAFOD web site (http://www.icdp-online.de/sites/sanandreas/index/)
for depths at which these samples were collected.

FORMATION FLUID SAMPLES (LIQUID AND GAS): Please specify the depths, types, and quantities of
formation fluids requested and how these samples should be provided (container type and material,
pressurized/unpressurized, delivery method, etc.). Contact the SAFOD Sample Manager Stephen Hickman
(hickman@usgs.gov) for information on sample availability, subsampling procedures, and delivery options.
