									How to fill in the COMPLAINT FORM

●   Fill in the date of the complaint at the top of the first page of the form.
●   In the square at the top of the first page write your name, your address and the telephone number at
    which you can be reached during the daytime.
●   In the square below enter the name of the authority and/or the public official you are complaining about.
    If you have received any case number or registration number from the authority you are complaining
    about, please enter it here as well.
●   On page 2 write a brief description of what you want to complain about and when the events took place.
●   Then go on to page 3 and describe what you consider was wrong with the way in which the
    authority/official acted.

Please note that you cannot write more than can fit in the space provided. If you continue, the beginning of your
description will “disappear”. If the space provided is too small, you must continue your description under the
heading “Additional information” on page 4.

When this Complaint form has been completed it can be submitted directly by e-mail by clicking the
