This is a sample proposal and questionnaire a graphic designer would give a prospective client looking for graphic design services. Depending on what the designer has already talked about with the client, it maybe appropriate to send the questionnaire after the proposal so that the designer can write the questions that pertain to the client's requests.

The designer will work with Client Company to establish the design criteria that will guide the process of creating an identity system and website. Information will be gathered from personal interview(s) and a brief questionnaire. Through this process we will identify specific communication and aesthetic criteria.
questionnaire & the creative brief
Before beginning design work, the project’s functions and goals must be identified. To help clarify these objectives, we first define Client Company’s target audience, core message and theme, as well as the company’s “personality.” The designer will provide a written synopsis of these findings for the client to review and approve in the form of a creative brief. This information will provide the criteria which will guide the design process.
general process: website
Concepting The designer will present three initial website concepts to Client Company. Design development Following client approval of one of the three concepts, and upon receipt of all final copywriting, the designer and client will begin a two-round presentation/review process based on the chosen concept. Changes will be discussed and agreed upon at each presentation. Each following presentation will incorporate all changes from the previous presentation. Production Once the design development is complete, the designer can begin building the website. A presentation of a working website will be shown. At this presentation the client and designer will agree on any last remaining changes. Completion Once the changes from the previous presentation are incorporated and reviewed the project will be considered finished.

the importance of design
The quality of the design and production should encourage conscious action on the part of Client Company’s target audience. Quality and clarity become both a form of promotion and an expression of Client Company’s goals. The designer will work in collaboration with Client Company to ensure that all materials produced support this goal.
client & designer partnership
The creation of an identity system and website is a collaboration between the designer and the client. It is understood that the length of the project depends on the timeliness of response to communications by both the designer and the client. In order to begin design work, it is expected that a completed questionnaire and any applicable client materials (e.g. photographs, copywriting, etc.) are presented to the designer.
budget estimates
Estimates are based on the designer’s estimated hours times her hourly rate, anticipated expenses and an efficient review and approval process on the part of the client.
terms of payment
Invoices will be presented every two weeks with expected payment due three weeks after date of receipt. The designer will at no time exceed estimated fees without client approval.

You must register a domain name and purchase hosting service before the site can be implemented. As the designer, I do not typically provide webhost services. I recommend clients purchase their domain name and hosting and then provide the login information to me in order to implement your site. This way, you maintain long-term control and ownership of your site. I can recommend web hosting services that may fit your needs.
