received), which was purchased (Tell where and when). The problem is (Tell why
you are unhappy Is it because the item does not work, does not work the way you
expected it to work, or something else is wrong?). I have tried to resolve the problem
by (Tell what actions you have taken, whom you talked to, and what happened).

I would very much like your help by (Say what you want to happen in order to make
you happy Do you want a refund or do you want another product? Give one or two
reasons why you want what you asked for).

Enclosed are copies of (Indicate sales receipt, invoice, repair ticket) of the product
or service. I look forward to receiving your reply by (Give a date 2 or 3 weeks from
the day you are writing the letter). If I do not hear from you by (List the date you
stated earlier), I will have no choice but to contact the government consumer

complaint. Tell exactly why you are complaining -- Is the price wrong? Did you not
charge the item? Was your card stolen or lost?)

  (Send this letter immediately after the unsatisfactory purchase, and forward a copy to the credit card
                             issuer along with a letter simil ar to Sample #4 .)

I am writing to complain about a charge from (List name of merchant) that
appeared on my recent (List name of credit card company) bill. This bill is incorrect.
I am asking (List name of merchant) to credit my account for (Give amount of credit
you are asking for).

The reason for this request is (Give reason -- Briefly and clearly give your side of the
situation). I expect that (List name of merchant) will credit my credit card account in
the amount of (Put the amount here) as soon as possible. Thank you for your
cooperation in this matter.

I am also officially requesting my credit card issuer to investigate this matter and
credit my account according to my rights under the Fair Credit Reporting Act.
The FCRA and the contract between (List name of merchant and the name of the
credit card company) requires no payment while the account is being challenged.

my recent bill. The amount of (Give dollar amount here) that appeared on my bill
with a date of (Give date here) is incorrect because the goods were not delivered
in the condition agreed upon. As you can see from the enclosed letter, I have
already attempted to get (List name of merchant) to credit my account for (Give
amount of credit desired).

(Briefly and clearly explain your side of the situation.)

I expect that (List name of credit card company) will immediately credit my account
for the challenged amount and, remove any interest assessed on the charge. I
have asked for an investigation of the matter and look forward to a letter
