This form will serve as your "contract" to complete 10 or more hours of mentored
activities for the Preparing Future Faculty in Engineering program. Completion of these
activities will be necessary for you to earn an "S" in 20-ECES-953. Please fill out this
form, have it approved by your PFF mentor, sign it, and bring it to Prof. Purdy for
approval before beginning your mentored teaching activities.

Notes:
1. You should plan to complete at least 6 hours of activities 1-2 (not counting class
preparation) and at least 1 hour of activity 4.
2. In addition to these 10 hours of activities, we also hope to have a Mentoring
Workshop this quarter. Your Mentor is encouraged to attend the workshop with you.
