							                      LETTER OF RECOMMENDATION QUESTIONNAIRE

If you are applying to private schools, or are requesting letters of recommendation from
teachers, you need to complete this Questionnaire and give it to your counselor and/or teachers.
Try not to repeat these responses in the same way you have completed them for the college
application, and think about tailoring the form slightly differently for each teacher. For example,
if you are asking a Science teacher for a recommendation, help he/she remember key projects in
classes you have taken that you would like them to highlight, which may be different from
projects/discussions you would like your English teacher to recall. Remember the key about the
letter: it gives admission representatives information about you that they don’t already have!
