SUBJECT: Availability of Revised FERS Refund Application Forms, SF 3106 and
         SF 3106A

The Office of Personnel Management has revised Standard Form (SF) 3106, Application for
Refund of Retirement Deductions Federal Employees Retirement System, and SF 3106A,
Current/Former Spouse's Notification of Application for Refund of Retirement Deductions
under the Federal Employees Retirement System. These forms are used by former FERS
employees to apply for a refund of retirement deductions. The new editions of the SF 3106
and SF 3106A, dated March 1996, are now available. The new version of the SF 3106
includes information concerning procedures to follow to have any taxable interest paid with a
refund rolled over directly to an Individual Retirement Arrangement (IRA), as required in
Public Law 102-318.

The SF 3106 includes a copy of the SF 3106A. Separate copies of the SF 3106A should be
ordered when additional copies of the spousal notification are needed. The SF 3106/3106A
can be ordered from the GSA Federal Supply Service using the normal
FEDSTRIP/MILSTRIP procedures.
