First payment ($200) at registration; Second payment Mar. 1st; Third payment Apr. 1st. (First payment must be made before student may begin lessons – No credit given for returning students who register late) NO STUDENT WILL BE ALLOWED TO BEGIN THE TERM UNTIL REGISTRATION IS COMPLETE AND TUITION HAS BEEN RECEIVED. I assume full responsibility of this Conservatory account, and if default be made, or any part thereof, then at the option of Birmingham-Southern College or its agent, the full amount owed shall at once become due and collectible, without notice. If in the judgement of Birmingham-Southern College it becomes necessary to engage the services of a collection agency or attorney to effect collection or to settle any dispute in connection with this account, the undersigned agrees to pay such collection agency or attorney fees, reasonable expenses, and costs hereby incurred. Signature of parent/guardian responsible party/student (if over 18 yrs old)Mail this registration agreement, tuition payable and signed policies and procedures statement to: BIRMINGHAM-SOUTHERN COLLEGE CONSERVATORY, BOX 549033, BIRMINGHAM, AL 35254 Conservatory office: 226-4960 Birmingham-Southern Conservatory POLICIES AND PROCEDURES REGISTRATION A completed registration form and a $200.00 initial payment is a prerequisite for participation in the Conservatory program for each term. Continuing students will receive registration forms prior to the start of each semester. No student will be allowed to begin lessons until a registration form, signed policies and procedures statement, and tuition payment has been received by the Conservatory office. If a returning student must begin late in the term due to late payment, no credit will be given for lessons lost due to late payment. All students must register EACH term. TERMS OF PAYMENT You may choose to pay in full or in 3 installments. A registration fee of $35.00 is due with the first payment for all new students. Tuition payments are due as follows. First Payment ($200.00) due at registration Second Payment due on October 1st for fall term /March 1st for spring term Third Payment due on November 1st for fall term/April 1st for spring term Balances must be received on or before due date. Collection activity will begin on past due accounts. Under no circumstances will a student be allowed to carry an unpaid balance into the next session. No student will be permitted to attend the Conservatory until the account is brought up to date and payment for any future terms will have to be received in full before the student will be allowed to register, no partial payments will be accepted. If, in the judgement of Birmingham-Southern College it becomes necessary to engage the services of a collection agency or attorney to effect collection or to settle any dispute in connection with the above terms, the individual who signs the registration form agrees to pay such collection agency or attorney fees, reasonable expenses, and costs as incurred. The individual who signs the registration forms understands and agrees to these terms. MAKE ALL TUITION PAYMENTS WITH CHECK OR MONEY ORDER. TUITION PAYMENTS MADE BY CASH OR VISA/MASTERCARD WILL HAVE TO BE MADE IN THE STUDENT SERVICES OFFICE. THE CONSERVATORY OFFICE CANNOT ACCEPT CASH PAYMENTS. THERE WILL BE A $10.00 FEE FOR RETURNED CHECKS. Withdrawals and Refunds NO ADJUSTMENT OF CHARGES OR REFUNDS WILL BE MADE IN THE CASE OF WITHDRAWAL AFTER LESSONS HAVE BEGUN. Once you register for the term and begin lessons you will have to pay for the entire term even if you stop attending lessons. A student should not register for lessons if he/she believes that there may be a reason for withdrawal later in the semester. Inability to find time for practice, changing of one's mind or unexpected changes in the student's schedule are not acceptable reasons for withdrawal. Only in the case of prolonged illness or moving away from the area will a withdrawal be considered. Failure to attend lessons once classes have begun does not constitute a withdrawal. ABSENCES Regular class attendance is expected of all students. A student who must miss a class or lesson should notify the Conservatory office, and the office will relay the information to the appropriate faculty member. Classes and lessons cannot be made up. Lessons will be made up in the case of a prolonged illness or death in the immediate family. There are no exceptions to this rule. If it is necessary for a teacher to miss a lesson or a class, students will be notified as far in advance as possible. Lessons will be rescheduled at a mutually convenient time. In case of inclement weather, listen to your local radio station for the closing of Birmingham-Southern College. Lessons will be made up if the College is closed. STUDENT EVALUATIONS Written reports and informal parent/teacher conferences serve to evaluate the student's progress. The written reports are intended as a measurement of each student's progress during the fall and spring term.
