Important: United States immigration regulations require that certification of admission be based upon both academic acceptance
and satisfactory evidence of adequate funds to meet all expenses that will be incurred during the student’s proposed course of study.
Please complete ALL ITEMS (front and back) on this form, carefully, in English. Use typewriter or print neatly in ink. Incomplete
forms will be returned unprocessed. Graduate applicants must submit this form even though an assistantship has been requested
(see reverse).
Please send documents to support your financial declaration. Supporting documentation is needed for each person or party who will
be contributing to the cost of your education. The documents should reflect the person’s financial status over time. All
documentation should be in English or be accompanied by an English translation. Examples of supporting documentation include:
several months’ bank statements or an official letter, on bank stationery, indicating the amount of funds available. All documents
must be officially translated into English and the monetary amounts expressed in U.S. dollars. Students need to keep a duplicate set of
all documentation. They will be necessary to apply for a visa and to enter the United States.

The costs listed above are approximate costs for one academic year (9 months). Add approximately $4,500.00 for summer living
expenses. Transportation costs are not included. If you are bringing family members with you, you must have approximately
$6,000.00 in additional support per dependent to cover the increased cost of room, board, and insurance.
Residents of countries which hold approved Sister City/Sister Province agreements, qualify for resident tuition. Undergraduate
resident tuition is approximately $3,805.00. Graduate resident tuition is approximately $6,085.00 for the nine-month academic year.
