                       Prepare in triplicate. Distribute original to employee, duplicate to HRS and triplicate to Field Personnel Folder

 A. The Department of Mental Health "Employee Management Performance System" directive requires that a "Warning Notice of
 Substandard Performance" be given to a permanent employee any time the job performance falls below the written success criteria. An
 overall performance evaluation which is "Below Performance Requirements" shall not be given or become a part of the employee's Official
 Personnel Folder unless a "Warning Notice of Substandard Performance" is on file in the office of Human Resource Services. The notice
 must contain an explanation that the employee has been informed of the essential or other job duty (ies) or objective(s) that is deficient, in
 writing, and what is required to correct the deficiencies.
 A "Warning Notice of Substandard Performance" may be given no less than 30 calendar days or more than 120 calendar days prior to the
 performance review date. An employee who receives a notice within 30 days of their annual review date shall have their annual review date
 advanced one (1) calendar day for each day the "Warning Notice" is in effect.

 B. Supervisors must complete the following information indicating the reason for the "Warning Notice of Substandard Performance:"

 1. The job duty(ies) or objective(s), is\are not being performed at the "meets" written success criteria and an explanation of the deficiencies
    must be given.

  with one of the following actions taken: reassignment, demotion, or termination.
