Proposal Requirements All proposals must be submitted in their entirety to United Way by 5:00 pm on November 14, 2007. Late or incomplete proposals will not be considered. One (1) hard copy and one (1) electronic copy of the proposal, including attachments are to be submitted by the due date and time. All proposals must include, at a minimum, the scope of services being requested and address how the desired outcomes are to be achieved (per the enclosed model description). Proposals may not exceed 10 pages (excluding attachments). The narrative should be double spaced with one inch margins and size 11 or 12 font. Proposals should include the following sections: RFP Cover Sheet (not part of 10 page limit) Initiative Abstract Organizational Capacity – Management and Governance Initiative/Program Plan Timeline for Implementation Community Collaborations Measurable Outcomes and Logic Model Initiative/Program Evaluation Initiative/Program Budget and Explanation of Budget Variances (not part of 10 page limit) Attachments (not part of 10 page limit)

III.
Evaluation Process All proposals will be evaluated by United Way’s Community Investment Cabinet with final decisions made by United Way’s Board of Directors. Applicants may be invited to meet with the selection committee to discuss their proposals on December 6 or December 7. A decision regarding who might be asked to present will be made on December 3; therefore, all agencies (or the collaborative’s designees) need to schedule time on December 6 or 7 in case they are requested to present.
IV.
Criteria for Selection Due to funding requirements, the applicant must be an existing United Way partner agency. However, partner agencies are allowed to sub-contract with, and provide funding for services, to a non-partner agency. [Note: Lead agencies may incorporate up to an additional 5% of the funds being subcontracted to non-partners to handle administrative requirements.] Collaboration with community partners is required. Proposals will be evaluated as follows: Initiative Abstract Organizational Capacity – Management and Governance Initiative/Program Plan --Best practice/evidence based components --How program/initiative addresses an underlying issue/s and the breakthrough opportunity --How well proposal meets identified outcome/s Community Collaborations Measurable Outcomes and Logic Model Budget Total 5 points 10 points 30 points

Proposal Information I. RFP Cover Sheet The proposal must include a completed cover sheet signed by an officer of the collaboration’s lead agency/ies. II. Initiative/Program Abstract Summarize the key elements of the collaborative’s proposal for the priority issue breakthrough opportunity in 200 words or less. III. Organizational Capability – Management and Governance Describe the collaborative’s capability to undertake the initiative. Address the relationship between the selected outcome and the collaborative partners’ missions, including past accomplishments. Describe the oversight of this program, including the collaborative’s management structure and governance. Please note whether a separate advisory board will be formed.

IV.
Initiative/Program Plan Describe how the initiative will be developed and implemented. (The plan must include, at a minimum, all of the elements outlined below.) Project Objectives: Description of the objectives of the project and how they will be achieved. This part of the Project Description should provide a narrative description of the objectives and activities included in the logic model. Addresses an underlying issue/s (root cause): Creating lasting changes requires addressing the causes of problems. Explain how this initiative “goes upstream” to address a problem’s root cause and is not simply another direct-service program. What strategies will the collaborative implement that address an underlying community condition? Evidence Based Practice: Provide a detailed discussion of the basis for claiming that the proposed project implements best practices – Use Attachment “B” to show references highlighting research on evidence based projects. Target population: Briefly describe the target population of project activities including cultural/gender specific information and estimated number served. Applicants’ Capacity: Description of the capability & experience of the applicant organization(s) with similar projects and populations. Describe the need and role of volunteers, in-kind resources, and technology (quantify as much as possible).
V.
Timeline for Implementation and achievement of outcomes Describe or provide the timeline for initiative/program development and implementation.

Community Collaborations For purposes of this RFP, collaboration is defined as: Two or more agencies working together to achieve common outcomes. Characteristics may include the following elements: joint leadership and governance, shared resources and other assets, a common budget and clear distinction of roles and responsibilities. Collaboration among partners should demonstrate a plan and system of coordination. Describe collaborations the organization plans to develop/enhance that will assist in the creation and operation of the initiative/program. List all partners and what their role is in the project. For any collaboration described in the proposal, the applying agency/ies should submit a memorandum of understanding (MOU) which describes the responsibilities of each party. Also describe here existing or potential leveraged resources: another grant, donations, volunteers.

VII.
Measurable Outcomes and Logic Model The proposal must address the sequence of activities needed in order to achieve the initiative/program outcomes and indicators specified in the model description, including data sources and data collection methods. Describe how the outcome results will be used to improve the program. Include a completed logic model, using the form provided in this RFP. The proposal may include the measurement of other outcomes in addition to those in the priority issue Theory of Change; however, those outcomes in the RFP must be central to the outcomes achieved.
VIII. Initiative/Program Evaluation Describe how the program will be evaluated internally for possible improvement and how the evaluation process will be implemented. Progress reports will be required at a minimum of every six months. However, depending on the nature and length of the initiative, reports may be required more often. IX. Initiative/Program Budget Provide a line item budget for each year if submitting a multi-year initiative, using the Collaborative Financial Report included in this RFP. On a separate page please explain any increase or decrease in revenue or expenses of 5% or more from year to year. X. Attachments Please include the following documents as Attachments: A.
