How do I request an      You must first obtain an application for a mail ballot from your local Board of Canvassers. To

                         pick one up for you or call your local Board and request that an application be sent to you. This
                         application must be filled out, witnessed or notarized by two witnesses and delivered to the

                         primary in which you wish to vote. The application may be mailed, picked up by you or person-
                         ally delivered by a person acting at your request.
