         YOU ARE HEREBY COMMANDED to appear before a person authorized to take depositions at the Office of the
Public Defender, 700 East Twiggs Street, 5th Floor, Tampa, Florida 33602, on Thursday, 03/16/2006 at 8 5 0 AM for the
taking of your deposition in this action. You are also to bring the following documents with you: Anyla11 records
pertaining to the ORJIFRJ Transportation, Movement, and Location of David Onstott, Booking #05023899, DOB:
07/08/1968, from April 12 through April 17,2005. If there is any fee for the copies of the documents requested you must
obtain approval for this expense prior to providing the copies. This office requires monthly billing for services rendered.
If the exact cost is not known, you may submit an estimate for approval.

        If you fail to appear or comply, you may be in contempt of court or the sanctions of the Court may be invoked.

        You are subpoenaed to appear and bring the above documents regarding the above-styled cause, by the following
attorney and have seven (7) days from service date to contact said attorney for any requests for rescheduling due to valid
conflict(s). Unless excused from this subpoena by these attorneys or the Court, you shall respond to this subpoena as directed.
If you need an interpreter, please call SAMANTHA L. WARD at (813) 307-4006 prior to the deposition date.

          Upon receipt of this subpoena and prior to the court date above, contact the Public Defender's Office witness coordinator at
(813)277-1502 to confirm the date and to receive notification of any changes, continuances, or special instructions. Pursuant to Florida
Statute 92.142, witnesses are entitled to receive a $5 attendance fee and mileage reimbursement for eachday's actual attendance. In
order to receive reimbursement, after the appearance, witnesses must turn in this subpoena, signed by the assistant public defender, to
the receptionist at 700 E. Twiggs St, 5th floor. Reimbursement will be made in the form of a check, and will be mailed by the Clerk of
Circuit Court within three to four weeks.
