In an interview, Paul Grundy, director of health-care technology and strategic initiatives for IBM Corp, talked about what kind of change medical homes represent and what IBM is doing. Grundy said that each patient has an ongoing relationship with a personal physician. Its a team approach. The personal physician leads a team of individuals who provide care. There is quality and safety in place. There's technology -- the electronic medical record, and evidence-based decision making that's underneath this.
