 ST STEPHENS EPISCOPAL CHURCH CENTENNIAL
 FEBRUARY 13, 14, & 15, 2010
 SHOW HOURS 9AM – 6PM




                               VENDOR APPLICATION / AGREEMENT

As a vendor applicant, we ask that you please include with your signed application, two (2) separate checks for
your $400 booth and $30 application fee, photos of your product and display, proof of insurance, and a Self
Addressed Stamped Envelope (SASE) for returned materials.

Whereas the vendor applies to occupy a 10' X 10' space / or cart space on said premises subject to the terms
and conditions contained herein and as attached, and will submit a photo of booth set-up and products for
selection by committee.

   1. Vendor agrees to have preparation areas set up by 9:00 AM all three days (Sat. - Feb. 13, Sun. Feb 14,
      and Mon. Feb. 15). Set-up will be available Friday, Feb. 12, from 12 - 7 PM. Entrance packages will be
      mailed in January, 2010 specifying your booth space and set up time.
   2. Vendor agrees to include with this application, a contractual booth fee of $ 400.00 plus, a non-
      refundable application fee of $30.00 with early-bird postmark deadline of November 1, 2009 and
      $50 with postmark deadline of December 1 st. Upon receipt, the processing check will be deposited,
      and the booth check will be held until accepted.
   3. Booth fee will be returned to denied applicants.
   4. Vendor agrees to obtain proof of liability insurance in the amount of $ 500,000 for edible items or skin
      products. Upon acceptance, the vendor must name "St. Stephen's Church Art and Craft Show" as
      "Additional Insured" on insurance policy & provide a copy of same with the signed vending contract.
   5. Vendor agrees to sell only the items approved by the committee and listed at the specified prices. No
      sale or discount signs allowed.

   6. The vendor and all employees must be properly attired at all times. Vendors and employees will not
       be permitted to drink alcoholic beverages during Festival hours.
   7. Electricity available only through coordination with director for an additional $ 75 fee. Gas fueled
       electrical generators will not be allowed without the permission of the director. If allowed, all
       generators must be equipped with sound barriers and all safety requirements be followed.
   8. Each vendor must individually comply with all State of Florida tax regulations as well as all local
       licenses, fees and permits prior to show.
   9. A copy of this agreement signed by both parties is notification of acceptance to the show.
   10. The vendor will be solely responsible for the security and safety of vendor's personnel, cash,
       equipment, goods, inventory, supplies, or other property.
   11. Vendor agrees to conform with all rules and will follow instructions by authorized personnel of this
       event, or vendor will be asked to leave the event immediately, forfeiting refund of rental space fee.
   12. No refunds will be made for cancellation after acceptance into the show.
   13. 24-Hour security will be provided Friday, Saturday, & Sunday nights.

                             VENDOR APPLICATION /AGREEMENT

Vendor herein waives, releases, and discharges any and all claims for personal injury, illness or
death, property damage or loss, which may result directly or indirectly from my/our participation.
This release is intended to discharge in advance the St. Stephen's Church, promoters, coordinator,
directors, producers, and sponsors of this event.
