                   APPENDIX V. QUITCLAIM DEED
                           DEED OF QUITCLAIM DEED AND RELEASE
         KNOW ALL MEN BY THESE PRESENTS, that the UNITED STATES OF AMERICA,
acting by and through the Secretary of the Air Force (the "Secretary" or the “Government”), under the
authority contained in Title 10, United States Code, Section 2878, and for the consideration contained in
Department of the Air Force Lease of Property on ______Air Force Base, ______, dated___________,
20__ (the "Lease") hereby transfers to________________, a limited liability company, duly organized
and existing under and by virtue of the laws of the state of __________with its principal
at______________________,___________ (the "Grantee"), in an "AS IS" and "WHERE IS" condition
without representation, warranty, or guarantee as to quality, quantity, character, condition, size or kind or
that the same is in a condition or fit to be used for the purpose for which intended, all right, title and
interest of the United States of America in and to certain family housing units and ancillary improvements
located on lands at _______Air Force Base, _______, as delineated in Schedule "A" and described in
Schedule "B", both attached hereto and made a part hereof.


        THIS INSTRUMENT NEITHER QUITCLAIMS NOR CONVEYS                                                ANY
        INTEREST IN THE LAND UNDERLYING THE IMPROVEMENTS

        This transfer is subject to the following covenants and conditions, which the Grantee, by
acceptance of this Quitclaim Deed, assumes for itself and its successors and assigns:


       1. This transfer is made subject to the terms and conditions of the Lease, including the
requirement that under certain conditions set forth in the Lease, whether by expiration of the
term or otherwise, title to all improvements shall be transferred to and become the sole and
absolute property of the Government without compensatio n thereof, and/or the improvements
shall be removed and the land restored at no expense to the Government, as provided in the
Lease.

        2. This transfer is made with the Grantee's full knowledge that all of the transferred
housing units were constructed in the 1950’s, 1960’s, 1970’s and 1990’s and that they may
contain lead based paint. Additionally, radon gas, and asbestos containing materials (ACM) have
been detected within a sampling of housing units. Grantee, by accepting this Quitclaim Deed,
hereby acknowledges the presence of lead based paint, radon gas, and ACM and has executed
the certificate attached hereto as Schedule "C."

Notary Public, personally appeared Fred W. Kuhn, personally known to me to be the person whose name
is subscribed to the foregoing Quit Claim Deed, and personally known to me to be the Deputy Assistant
Secretary of the Air Force (Installations), and acknowledged that the same was the act and deed of the
Secretary of the Air Force and that he executed the same as the act of the Secretary of the Air Force.
