How to Claim Your FREE LAMP: 1. To receive your FREE LAMP, purchase a new Panasonic PT-DW5000U/UL projector from a dealer or reseller in the United States between June 29, 2006 and October 31, 2006. 2. Return this completed form, including the serial number label, along with a copy of your invoice with the purchase price circled. IMPORTANT: You must cut out and include the original label from the carton, which includes the UPC code, model number and serial number, or your claim cannot be processed. 3. Offer valid for purchase made from June 29, 2006 thru October 31, 2006. All claims must be postmarked within one month of purchase. 4. This offer is only available to the end user and original purchaser. No claims from re-sellers, dealers/distributors, their employees or warehouse facilities, groups, clubs or organizations will be accepted or acknowledged. 5. Product must be new and un-used. No B-Stock Product allowed. 6. Offer void where taxed, prohibited or restricted by law. Offer good only in the 50 United States. 7.
