4-H PERSONAL DEVELOPMENT REPORT
INTRODUCTION
This form is designed to help you fill out your Annual Achievement Report. Some items may
apply to more than one category on the Achievement Report. Wherever the item is recorded, you can
only use it once per year. You cannot count any item until you have completed it.
Remember that this report is a continuing record and you should add the previous year's total to it
each year as directed on the final page. Since this report also combines the Annual and Permanent
Records as well as the Rank Sheet, maintain it even if you do not apply for a star. Also maintain this
report after you attain a Gold Star since you can use much of the material in it to fill out national report
forms, scholarship applications and other award forms.
A. LOCAL 4-H CLUB MEETINGS ATTENDED
To figure the percent for this item, divide the total number of local club meetings attended by the total number of
local club meetings held. Move the decimal point two places to the right.If the number is less than 80, you cannot
qualify for a star under any circumstances. The local 4-H club determines an excused absence. Excused
absences count as meetings attended. Some kind of make-up activity is recommended...perhaps if the club
meeting is missed, the member could attend the club meeting of another club. (NOTE: Physical presence is
required at all meetings for a 100% attendance pin. Pre-authorized excused absences will qualify member for
100% attendance pin.)

To get credit as a 4-H member, you must complete at least one project, turn in a Personal Development Report, Project Report
and Project Picture or Story pages. Member will not earn a stripe if these things have not been done. Refer to your club
bylaws for local club rules.
