The undersigned, on behalf of himself/herself, heirs, successors and assigns, being the parent of
________________________________________________________, in consideration of the
Independent School District No. 271’s approval of the trip for my child
______________________________________________ does hereby RELEASE AND
DISCHARGE Independent School District No. 271, and each of their respective officers,
directors, employees, and agents (the “Released Parties”) from ANY AND ALL CLAIMS,
DEMANDS, RIGHTS AND CAUSES OF ACTION (“Claims”) OF ANY KIND
WHATSOEVER WHICH THE UNDERSIGNED OR ANY OF ITS HEIRS, SUCCESSORS
AND DESIGNS NOW HAVE OR LATER MAY HAVE AGAINST ANY RELEASED PARTY
RESULTING FROM OR ARISING OUT OF_________________________________________

         The undersigned acknowledges and understands that this release EXTENDS TO AND
RELEASES AND DISCHARGES ANY AND ALL CLAIMS that the undersigned or any of the
heirs, successors and assigns have or may have against the Released Parties or any of them
resulting out of the trip described above, including without limitation all such claims resulting
from the NEGLIGENCE of any Released Party or arising from STRICT LIABILITY, and
regardless whether such Claims now exist or hereafter arise, or are known or unknown,
contingent or absolute, liquidated or unliquidated, foreseen or unforeseen, or arise by operation of
law or otherwise.

        Also, the undersigned acknowledges and understands that by signing this Release, the
undersigned by his/her heirs, successors and assigns AGREE NOT TO SUE any or all of the
Released Parties for any injury or death to my child or for any damage to of destruction of his/her
property.

        By signing this Release, the undersigned certifies that the undersigned has read this
Release and fully understands it, and that the undersigned is not relying on any statement of
representation of any of the Released Parties in executing this Release. This Release is freely and
voluntarily executed by the undersigned.

         The undersigned further agrees to indemnify and hold harmless the Independent School
District No. 271 hereinafter termed “school”, its officers, volunteers, employees and agents, from
all claims, causes of action, lawsuits, damages, losses or expenses, including attorney’s fees,
arising out of or resulting from school’s (including its officers, agents, volunteers or employees)
performance of any acts relating to the trip to _______________________________________on
or about ___________________________________(dates and year) provided that any such
claim, damages, loss or expense is attributable to body injury, sickness, disease or death, or to an
injury to or destruction of property including the use resulting therefrom and is caused by any
negligent act or omission or willful misconduct of the school, including its officials, agents,
volunteers or employees.

         To the extent that the scope of this Release and Indemnification is unenforceable in any
jurisdiction, such scope will, or to such jurisdiction only, be automatically limited to the extent
necessary to make this Release and Indemnification enforceable in such jurisdiction, without
invalidating any other portion of the Release and Indemnification.

        THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.
